PF方案联合放疗与常规放疗N_2、N_3期鼻咽癌的近期疗效比较

摘　　要：目的：比较放、化疗与单纯放射治疗 Ｎ２、Ｎ３期鼻咽癌的疗效及毒副作用。方法：１２６例 Ｎ２、Ｎ３期鼻咽癌随机分入放、化疗组（６３例）及单纯放疗组（６３例）。结果：放、化疗组及单放组放疗后３个月鼻咽部肿瘤全消率分别为 ９３．６％和 ８５．７％（Ｐ＞０．０５），颈淋巴结全消率分别为 ８４．１％和 ５７．１％（Ｐ＜０．０５），ＣＴ扫描检查鼻咽肿瘤完全消退率分别为７１．４％时５０．８％（Ｐ＜０．０５），放、化疗组口腔炎、恶心、呕吐较单放组多见且明显（Ｐ＜０．０５）。放、化疗组血液毒性大于单放组（Ｐ＜０．０５），但可接受。结论：ＰＦ方案联合放射治疗Ｎ２、Ｎ３期鼻咽癌有较好的局部控制率和近期疗效。Objective: To evaluated the result of Radiotherapy combined with chemotherapy for NPC (stage N2、 N3) . Methods: 126 patients (pta) with locally advanced NW (stage N2、 N3) were divided into two groups. Radichemotherary group: 63 canes, in which previously untreated NW with locally advanced NW received of 65-70 Gy/6 .5-7 week to nasopharynx and 65-70 Gy/6.5-7 week to neck and 2-3 cycles of chemotherapy with PF protocol (5.-FIJ and cisplatin) . Radiotherapy group: received the same radiotherapy alone. Results: The primary tumor control rate of radiochemotherapy group and radiotherapy group at 3 months after radiotherapy were 93.6% to 85.7% (P >0.05), the cervical lymph nodes control rate were 84.1% to 57.1% (P <0.05), the primary tumor CT control rate were 71.4% to 50.8% respectively (P <0.05) .There were more mucositis, nausea, vomiting and more bone marrow in radiochemotherapy (P <0.05) . Conclusion: These data showed radiotherapy plus chemotherapy was favorable in the treatment of local advanced NPC (stage N2 , N3) and showed increase local disease control.
