高血压脑出血患者肿瘤坏死因子水平与胰岛素抵抗的关系

摘　　要：目的 :为探讨高血压脑出血 (HCH)患者血清肿瘤坏死因子 - α(TNF- α)水平变化及其与胰岛素抵抗 (IR)的关系。方法 :采用氧化酶法、放免法和酶联免疫法分别测定 2 3例 HCH患者 (A组 )、 2 5例高血压患者 (B组 )、 2 1例正常健康人 (C组 )的空腹血糖 (FBG)、空腹胰岛素 (FINS)及血清 TNF- α值 ,并计算胰岛素敏感指数 (ISI)。结果 :A组较 B组、C组 :FINS、TNF- α水平较显著增高 (P<0 .0 1) ,而 ISI较显著降低 (P<0 .0 1)。相关分析发现 ,A组 TNF-α与 ISI水平呈显著负相关 (r=- 0 .5 9) ,同 FINS呈显著正相关 (r=0 .5 5 )。结论 :HCH患者 TNF-α水平增高 ,且与胰岛素抵抗并存 。Objective:To investigate the changes of tumor necrosis factor\|α(TNF α) in patients with hypertension complicated cerebral hemorrhage(HCH) and the relationship between TNF α and insulin resistance(IR).Methods:Twenty\|three patients with HCH(group A);25 patients with hypertension (group B);21 healthy peoples(group C) were selected to compare fasting blood\|glucose(FBG), fasting blood\|insulin (FINS) and serum TNF α levels by oxidase method, RIA and ELISA, insulin sensitivity index(ISI) was also calculated.Results:Analysis of variance revealed that FINS, TNF α levels were higher( P <0 01) while ISI was lower( P <0 01) in group A than those in group B and group C.Correlation analysis showed that TNF α level in HCH patients was negatively correlated to ISI( r= -0 59), and positively correlated with FINS( r= 0 55).Conclusion:The results indicate that higher level of TNF α and IR may play an important role in development of hypertension complicated cerebral hemorrhage.
