特发性眼眶炎性假瘤的临床诊断与治疗

摘　　要：目的 回顾性分析眼眶炎性假瘤的临床论断、并发症及CT或MRI对其定位诊断分型意义的探讨。方法 总结分析我院10年间资料完整的眼眶假瘤45例(48眼)。结果 前部型假瘤12例(12眼),痊愈8眼,好转4眼;后部型假瘤12例(12眼),痊愈3眼,好转9眼,其中继发青光眼2眼,视神经萎缩2眼,眶炎综合征1眼;弥漫型6例(7眼),痊愈3眼,其中继发青光眼3眼,视神经萎缩2眼;泪腺炎型7例(7眼),痊愈3眼,好转4眼,继发青光眼1眼;肌炎型8例(10眼),痊愈8眼,好转2眼,继发青光眼3眼。平均总治愈率(26眼)54.17％。入院前平均视力0.17,出院平均视力0.29。结论 高度重视对眼眶炎性假瘤的积极治疗,减少误诊及并发症,CT或MRI对眼眶炎性假瘤的定位诊断分型对指导治疗、评估疗效具有重要意义。Objective To analyze retrospectively clinical diagnosis complications of orbital inflammatory pseudotumor and to discuss the management, vision and complications by CT or MRI classification. Methods Forty-five cases (48 eyes) with orbital pseudotumors were analyzed in past years. Results Inflammatory pseudotumors were classified into five types, Anterior pseudotumor 12 cases, 8 patients obtained a cure, 4 patients got a partial cure with no recurrence; Posterior pseudotumor12 cases, 3 patients obtained a cure, 9 patients got a partial cure, which of 2 patients with high intraocular tension, with optic nerve atrophy 2 patients with orbital apex syndrome and 1 patients. Diffuse pseudotumor 6 cases (7 eyes), 3 patients (4 eyes) obtained a cure, 3 patients got a partial cure, which of 3 patients (3 eyes) with high intraocular tension, and 3 patients (3 eyes) with optic nerve atrophy. Dacryadenitis 7 cases, 3 patients obtained a cure, 4 patients got a partial cure, 1 patients with high intraocular tension. Myositis 8 patients (10 eyes), 6 patients (8 eyes) obtained a cure, 2 patients got a partial cure, 3 patients (3 eyes) with high intraocular tension and 24 patients (26 eyes) were cured (54.17%), average visual 0.17 before hospitalization and average vision 0.29 out of hospital. Conclusion It is very important to treat timely orbital inflammatory pseudotumor to reduce the mistaking diagnosis and complications, It is significant to treat and evaluate a cure according to five types of localized diagnosis classified by CT or MRI.
