摘　　要：目的 探讨利用角膜地形图引导切口使白内障术前存在的角膜散光通过白内障手术达到矫正或改善以便获得较好的裸眼视力。方法 在角膜地形图陡峭轴做散光选择切口进行超声乳化白内障吸出注人器法人工晶体植入术。结果 917例(1226只眼)患者术后散光状况均较术前有明显改善。本组病例术前散光<1.50D者术后平均散光0.51±0.10D。术前1.75～2.75D经一对弧形角膜切口术后75％小于1.50D,平均0.85±0.52D。术前3.00～5.00D术后平均1.75±0.70D,而>5.25D散光者术后平均散光2.75±0.90D。结论 角膜地形图引导白内障手术能较好的矫正散光,获得满意的视力。有条件的单位值得推广。Objective To study the clinical effects of cataract phacoemulsification with its incision decided by corneal topography system. Methods An astigmatic incision was made on the high degree axis decided by the corneal topography system, through which, cataract phacoemulsification was performed and foldable intraocular lens was implanted. Results In all the 917 cases (1226 eyes), astigmatism was corrected after surgery. For the cases whose pre-operative astigmatism was < 1.50 D, between 1.75~2.75 D, between 3.00-5.00 D and > 5.2 D, their average post-operative astigmatism improved to 0.51±0.10 D, 0.85±0.52 D, 1.75±0.70 D and 2.75± 0.90 D, respectively. Conclusion Cataract phacoemulsification with its incision decided by corneal topography system can better rectify astigmatism and achieve satisfactory vision. It is worth popularizing.
