摘 要:目的 探讨老年黄斑变性 (ARMD)视神经传递功能的损害。方法 按VEP标准 ,对ARMD 47例 ( 6 8眼 ,其中湿性型 14眼 )和对照组 2 9人 ( 35眼 )作图形或闪光 4类VEP记录。结果 ( 1)ARMD的VEP各指标与对照组比 ,除TN2 外 ,其余均呈显著 (P <0 0 5 )或非常显著 (P <0 0 1)改变 ;( 2 )湿性型与干性型的各类VEP异常率比较 ,均呈统计意义 (P <0 0 5或P <0 0 1) ;( 3)ARMD的图形类VEP与闪光VEP的异常率比较 ,P <0 0 1;而同属图形类VEP ,将P VEP与P On/off VEP和P S VEP比较异常率也呈明显差异 (P <0 0 1)。结论 ARMD确有视神经传递功能 (VEP)的损害 ,其以黄斑中心凹细神经纤维功能受累为主 ,随病情加重其累及程度更明显 。ObjectiveTo explore the functional damage of visual neural transmission in age related macular degeneration(ARMD).MethodsAccording to standard for visual evoked potentials,four sorts of VEP by pattern or flash stimuli were recorded in 47 ARMD(68 eyes,age:50~75) and 29(35 eyes) age matched countrol subjects.Results(1)all indexs of VEPs in ARMD were showed significant al terations( P <0 05 or P <0 01),except for TN 2 of F VEP( P >0 05) in comparison with the controls.(2)To divide ocular fundus of ARMD into two groups,the abnormal rates of exudative group were significantly higher ( P <0 05 or P <0 01) than those of atrophic group.(3)The abnomal rates of pattern stimuli were higher than those of flash stimuli,and P VEP possessed obvious difference ( P <0 01) in comparision with P On/off VEP and P S VEP under the same pattern stimuli.ConclusionThere certainly is functional damage of the visual neural transmission in ARMD,and the involvements is primarily the small neural fibres of macula fovea.But the neurophysiological abnormalities are more obvious with the serious degrees of disease,and the damage tends to developing toward fibres of the peripheral retina.
