十二指肠和小肠肿瘤P27mRNA,PTENmRNA表达及意义

摘　　要：研究十二指肠和小肠良恶性肿瘤组织中P2 7mRNA ,PTENmRNA表达及临床病理学意义。方法 :标本经 10 %福尔马林固定 ,常规石蜡包埋后连续切片 ,P2 7mRNA和PTENmRNA染色方法为原位杂交染色法。结果 :4 4例腺癌P2 7mRNA和PTENmRNA阳性率 (5 2 % ,5 7% )明显地低于 10例腺瘤 (90 % ,90 % ) ,P2 7mRNA和PTENmRNA阴性病例均为同 1例腺上皮呈重度不典型增生腺瘤。组织学分级Ⅰ级、癌细胞未侵袭肌层 +浅肌层、肿块最大径≤ 3cm和淋巴结未转移病例P2 7mRNA、PTENmRNA阳性率明显高于组织学分级Ⅲ级、癌细胞侵袭深肌层 +浆膜层、肿块最大径 >3cm和淋巴结转移病例。P2 7mRNA和PTENmRNA在腺癌中表达无明显相互关系。结论 :P2 7mRNA和PTENmRNA表达与十二指肠和小肠癌发生发展、生物学行为、转移发生及预后有密切关系 ,检测十二指肠和小肠良性病变P2 7mRNA或 /和PTENmRNA表达对预防和早期发现恶性肿瘤可能有重要临床应用意义。Objective:To study the expression of P27mRNA and its clinicopathological significances in the adenoma and adenocarcinomas of duodenum small intestine. Methods:The specimen were fixed in 1 0% formalin and routinely embedded in the paraffin.Then the specimen were contin uously sliced into pieces.The P27mRNA and PTENmRNA were stained by in situ hybri dization method.Results:The positive rates of P27mRNA and PTENmR NA were significantly lower in 44 cases of adenocarcinoma than those in 10 case s of adenoma (57% vs 90%,χ 2= 3.85 , P <0.05).The same case of the neg ativity of P27mRNA and PTENmRNA was found in the adenoma with severely-atypical hyperplasia.The positive rates of P27mRNA,P TENmRNA were significantly higher in the adenocarcioma of histologic grade Ⅰ, without infiliated muscular layer+superficial muscular layer,maxial diameter of mass≤ 3cm,and without lymphnode metastasis than that in one of histologic grad e Ⅲ,infiliated deep muscular layer+serous layer,maxial diameter of mass>3cm,an d with lymphnode metastasis.No correlation was found between the expression of P 27mRNA and PTENmRNA in adenocarcinoma.Conclusions:The expression of P27mRNA orPTENmRNA might be related to the carcinogenesis,progress,biologica l behaviors,lymphnode metastasis and prognosis of duodenum and small intestinal adenocarcinoma.The assay of P27mRNA or/and PTENmRNA in benign lesions of duodenum or small intestine might have important clinical values in the preve ntion and early-stage finding of the malignant neoplasms of duodenum or small in testine.
