食管癌刷检、活检和手术标本端粒酶活性检测的比较

摘　　要：目的　探讨端粒酶在食管癌发生、发展过程中的变化 ;刷检细胞能否取代组织标本进行端粒酶活性检测。方法　应用TRAP PCR ELISA定量法对 15 0例食管癌 ,130例慢性食管炎 ,10例食管不典型增生的内镜活检标本和刷检标本及 30例手术切除的癌组织及癌旁组织进行端粒酶活性检测并进行比较。结果　食管癌组织中端粒酶活性水平及阳性率明显高于食管炎和癌旁组织。食管不典型增生组织端粒酶活性水平及阳性率亦明显增高 ,且随着食管不典型增生程度的加重而不断升高 ,但食管癌与不典型增生组织之间端粒酶活性及阳性率无显著性差异。食管癌手术标本、活检标本、刷检细胞标本中端粒酶的阳性检出率以刷检细胞标本为高 ,但无显著性差异。结论　端粒酶的重新活化发生在食管癌的早期阶段 ,并且与恶性发展有明显关系 ,端粒酶的检测可以应用于食管早期癌的诊断。食管刷检细胞完全可以取代组织标本进行端粒酶活性检测 ,这为大规模普查食管癌工作提供了 1种简便方法。Objective To investigate the changes of telomerase activity in the carcinogenesis and progression of esophageal carcinoma(EC);and whether the brushed cells can substitute tissue specimen for detection of telomerase activity.Methods The brushed cells and biopsy tissue specimen were taken from 290 cases of esophageal lesion by endoscopy;surgical specimen of carcinoma tissue and adjacent normal tissue were taken from 30 cases of EC.The quantitative TRAP-PCR-ELISA was used to detect telomerase activity in the above specimen.Results Telomerase activity and positive rate were significantly higher in EC than in esophagitis and normal esophageal tissue.Telomerase activity and positive rate in esophageal atypical hyperplasia were elevated signficantly,and associated with the degree of atypical hyperplasia;however,it was not different between EC and atypical hyperplasia.The positive rate of telomerase activity was highest in the brushed cells among all specimen of EC,but there was no statistical difference.Conclusion The reactivation of telomerase activity happens in the early stage of EC,and associates with malignent progression.The detection of telomerase activity can be used as a tool for early diagnosis of EC.The brushed cells can substitute tissue specimen for detection of telomerase activity,and provide supportment for large scale screen of EC.
