摘  要:目的 研究钙磷脂结合蛋白Ⅰ (Annexin Ⅰ ,AX Ⅰ )在子宫内膜病变中的表达及在交界病变鉴别诊断中的意义。方法 收集 3 7份子宫内膜不同病变标本 ,经HE染色 ,在光镜下按国内及国际妇产科联合会 (FIGO)标准分类 ,AX Ⅰ、雌激素受体、孕激素受体、表皮生长因子受体、胰岛素样生长因子 Ⅰ受体与c erbB 2的免疫组织化学染色采用LSAB法。结果 腺囊性子宫内膜增殖症 12例、子宫内膜不典型增生 (ATH) 10例、子宫内膜癌 (EC) 15例。免疫组织化学AX Ⅰ在腺囊性增殖症中 ,除 1例弱阳性外 ,均为阴性 ;在不典型增生组 9例为强阳性 ,1例弱阳性 ;内膜癌则 7例弱阳性 ,8例阴性 ;内膜鳞化或鳞癌区呈强阳性。c erbB 2蛋白在不典型增生组 9例为阳性 ,1例阴性。表皮生长因子受体 (EGFR)均呈强阳性 ,胰岛素样生长因子受体 Ⅰ受体的表达弱且无规律。p5 3表达在EC组较ATH组略强。结论 AX Ⅰ基因蛋白可能在肿瘤的发病初期起作用 ,癌变后其表达下降 ,甚至消失 ;AX Ⅰ强阳性对ATH与EC鉴别具有一定价值。另外 ,c erbB 2与AX Ⅰ的表达似乎存在一种平行的关系。Objective To investigate expression of Annexin Ⅰ in various endometrial lesions and its significance in differential diagnosis of endometrial borderline lesions. Methods We collected 37 specimens with different endometrial lesions, and reclassified them according to China and FIGO′s standard, which included 12 cases of endometrial hyperplasia (EH), 10 cases of atypical hyperplasia (ATH) and 15 cases of endometrial carcinoma (EC). Immunohistochemical staining for AX Ⅰ, c erbB 2, p53 protein, ER, PR, EGF and IGF were performed by LSAB. Results The results showed that for AX Ⅰ, 1 cases of EH was weakly positive and others were negative; 9 cases of ATH showed strong positivity in the cytoplasm and the cell membrane, and 1 weakly positive; 7 cases of EC were positive and 8 negative. c erbB 2 was expressed in 9 cases of ATH and 1 was negative. Expressions of EGFR, ER and PR were strong in all cases, but IGFR was negative in almost all cases. P53 expression was higher in EC than in ATH. Conclusions AX Ⅰ expression suggests that (1) AX Ⅰ protein may play an important role in the pathogenesis of cancer, the expression decreased and disappeared after malignant change. (2) High expression for AX Ⅰ can be used to differentiate ATH and EC. In addition, c erbB 2 expression appeared to parallel the expression of AX Ⅰ in both ATH and EC, but in ATH it was higher than that in EC.
