口服大黄水对肝硬化患者血清TNF-α,IL-6水平的影响

摘　　要：目的　了解TNF -α ,IL -6对肝损害的作用 ,研究口服大黄水治疗对肝炎后肝硬化患者血清TNF -α ,IL-6的影响。方法　 47例乙型肝炎后肝硬化患者随机分为 2组 ,均予常规护肝治疗 ,并根据病情给予有效抗生素或支持治疗 ,治疗组加服大黄水 ,疗程 14d。治疗前后检测肝功能和血清TNF -α ,IL -6水平。结果　治疗前肝功能C级患者的TNF -α ,IL -6水平均明显高于A级患者 (P <0 .0 5 ) ,A ,B级差异无显著性 (P >0 .0 5 ) ;治疗前TNF -α与IL -6水平呈正相关 (r =0 .42 0 ) ,两者均与TB呈显著正相关 (r分别为 0 .46 2 ,0 .45 8) ,TNF -α与ALT水平呈正相关 (r =0 .30 1) ,但IL -6与ALT水平无关 (r =0 .198) ;治疗后随着TNF -α ,IL -6水平的降低 ,所有患者的临床表现均有改善 ,ALT ,TB水平均下降 ,治疗组TNF -α ,IL -6水平的降幅及病情的改善程度均优于对照组 (P <0 .0 5 )。结论　TNF -α ,IL -6在肝损害中起了重要的作用 ,口服大黄水有助于降低TNF -α ,ILObjective To research for liver injury effects of TNF-α and IL-6, and the effects of oral DaHuang water on serum levels of TNF-α, IL-6 in patients with posthepatitis liver cirrhosis. Methods 47 patients with posthepatitis B liver corrhosis were randomly divided into two groups. All patients were treated with liver safeguard, effective antibiotis or support medication based on the illness conditions. One group (treatment group) was added oral DaHuang water. Hepatic function test, measurement of serum TNF-α and IL-6 were performed before and after the therapy. Results The levels of TNF-α, IL-6 were significantly higher in patients with grade C liver function than in grade A before ( P< 0 05).There were no significantly different between patients with grades A and B liver funtion( P> 0.05). Levels of TNF-α positively correlated with IL-6 ( r= 0.420) before the therapy. Both significantly positively correlated with TB ( r= 0.462 and 0.458). ALT positively correlated TNF-α( r= 0.301), but not with related IL-6 ( r= 0.198). As the decreased of TNF-α and IL-6, patients' symption improved, ALT and TB decreased after the therapy, amplitude of decreased of TNF-α and IL-6, improvement of illness condition were better in treatment group ( P <0 05). Conclusion TNF-α and IL-6 play an importmant role in liver injury, oral DaHuang water is benific in reducing TNF-α and IL-6.
