摘 要:目的 探讨营养不良、肾病本身和糖皮质激素对大鼠肾脏IGF 1和IGFBPsmRNA表达的影响 ,阐明靶器官IGF 1和IGFBPs分泌紊乱与肾病综合征大鼠生长障碍的关系。方法 2 4只周龄相同体重相近的雄性SD大鼠被随机分成正常组、食物对照组、阿霉素 (5mg/kg)肾病组 (肾病组 )和地塞米松 [1 8mg/ (kg·d) ]治疗的阿霉素肾病组 (激素治疗组 )。血清GH和IGF 1浓度测定用放射免疫法 ,肾脏IGF 1和IGFBPsmRNA表达采用RT PCR法检测。肾脏GHR和IGF 1R检测采用放射性受体测定法 (RRA)。结果 (1)四组大鼠实验末时的鼻 尾长度及体重的增长按正常组、食物对照组、肾病组和激素治疗组依次减慢。 (2 )食物对照组大鼠血清GH浓度显著高于正常组 ,肾病组与正常组差异无显著意义 ,激素治疗组较肾病组显著下降。食物对照组和肾病组大鼠血清IGF 1较正常组明显下降 ,激素治疗组较肾病组进一步降低。 (3)食物对照组大鼠肾脏IGF 1AmRNA表达 (0 5 6 0± 0 0 35 )高于正常组 (0 2 18± 0 0 2 2 ) ,肾病组 (0 376± 0 0 2 7)也增高但却低于食物对照组 ,激素治疗组 (0 188±0 0 2 3)较肾病组明显降低。食物对照组大鼠肾脏IGFBP 2mRNA表达 (0 478± 0 0 19)与正常组(0 438± 0 0 38)差异无显著意义 ,肾病组 (0 316± 0 0 32 )Objective Growth failure was frequently observed in persistent nephrotic children The study aimed to investigate the effects of malnutrition, nephrosis and glucocorticoid therapy on renal insulin like growth factor (IGF) and insulin like growth factor binding protein (IGFBP) mRNA and to illustrate relationship between growth failure in nephrotic rats and turbulence of IGF 1 and IGFBPs autocrine/paracrine Methods Twenty four male Spraque Dawley (SD) rats were randomly divided into control, pair fed, doxorubincin induced (5 mg/kg) nephrotic (nephrotic) and dexamethasone treated [1 8 mg/(kg·d)] nephrotic (des treated) groups Growth hormone (GH) and IGF 1 concentration in serum were measured with radioimmunoassay (RIA) Renal IGF 1 and IGFBPs mRNA expressions were measured with reverse transcription polymerase chain reaction (RT PCR) Growth hormone receptor (GHR) and insulin like growth factor receptor 1 (IGF 1R) in kidney were assessed with radioreceptor assay (RRA) Results (1) The nose tail length in control, pair fed, nephrotic and des treated groups were 42 16±0 41, 41 16±0 41, 40 00±0 26 and 39 78±0 20 cm, and weights were 309± 9, 258± 4, 222±10 and 132±6 g, respectively, at the end of experiments In contrast to control rats, the increases of length and weight were slower in nephrotic rats ( P <0 01), the slowest in des treated rats ( P <0 01) (2) The serum GH level was increased in pair fed (1 62±0 22 μg/L) rats as compared with control rats (1 08±0 06 μg/L) ( P <0 01) There was no significant difference between nephrotic (1 40±0 07 μg/L) and control rats ( P >0 05) However, the GH level was decreased in des treated rats (0 90±0 08 μg/L) as compared with nephrotic rats ( P <0 01) The serum IGF 1 was reduced in pair fed (2 023±143 μg/L) and nephrotic (2 045±118 μg/L) rats as compared with controls (2 496±68 μg/L) ( P <0 01), and further decrease was seen in destreated rats (857±77 μg/L) ( P <0 01) (3
