老年迟发性癫癎发作的病因、临床表现及脑电图特征分析

摘　　要：目的：探讨老年迟发性癫 发作的病因、临床特点及脑电图特征。方法：对９８例老年迟发性癫 发作患者的病因、临床发作类型及治疗结果进行统计分析。结果：７３例有明确病因，分别为脑血管病４５例，脑肿瘤１６例，脑外伤５例，脑萎缩５例，血液病２例；临床发作为部分性发作６８例（单纯部分性发作４４例，复杂部分性发作２４例），全面性发作２６例，不能归类的发作４例；脑电图正常２１例，异常７７例，异常脑电图主要表现为低至中幅慢波增多，呈弥漫性或局限性分布，７７例异常脑电图中９例出现癫　样放电；临床疗效：完全控制４４例（４５％），部分控制２４例（２５％），无效１０例（１０％），死亡２０例（２０％）。结论：老年迟发性癫　大部分病因明确。临床以部分性发作为主，脑电图以慢波活动为背景，癫　样放电阳性率低，临床完全控制率低。Objective: To explore the cause of disease, clinical manifestations and EEG features Methods: Stastical analysis of cause of disease,clinical seizure types and therapeutic effect were done in 98 patients with late on- set epileptic seizures. Results: The cause of disease was determined in 73 patients,including 45 patients with cerebral vascular disease, 16 patients with brain tumor, 5 ones with brain trauma, 5 brain atrophy and 2 patients with blood dis- eases. The types of seizures included partial seizures in 68 patients (44 patients with simple partial seizures, and 24 ones with complex partial seizures) , generalized seizures in 26 ones, and unclassified seizures in 4 patients. Normal EEGs were found in 21 patients and abnormal in 77 ones, Abnormal EEGs mainly showed increase of slow waves with low to middle amplitude,with diffuse or local distribution . Epileptiform discharges were found only in 9 of the 77 ones; The outcome of treatment were as follows: Seizure free was found in 44 patients(45%), and partly controlled seizure in 24 ones (25%), no therapeutic effect was noted in 10 patients (10%) ,and death occurred in 20 ones (20%). Conclusions: Most of old patients with late onset epilepsy might have definite cause of disease . Clinical manifestations mainly show partial seizures . Slow waves might be the background of EEGs, and EEGs showed low positive rate of epileptiform discharges. The rate of complete seizure free might be low.
