摘 要:目的研究肝细胞癌组织及其癌旁非癌肝组织中Fas/FasL(Fas ligand)的表达情况及其与临床病理的关系。方法应用免疫组化Envision和RT-PCR双重方法检测30例原发性肝细胞癌(hepatocellular carcinoma,HCC)患者的癌组织及其癌旁非癌肝组织的Fas蛋白和FasL的mRNA及其蛋白的表达情况。结果 12例(12/30,40.0%)HCC表达Fas蛋白,明显低于癌旁组织(21/30,70.0%,P<0.05).21例(21/30,70.0%)HCC的FasL蛋白为阳性,略高于癌旁组织(17/30,56.7%),两者无显著性差别,但HCC的FasL mRNA水平较癌旁组织为高(70.1/32.2,P<0.05),且同一癌组织多呈Fas FasL’表型(16/30,53.3%),癌组织Fas与FasL表达显著相关。HCC中Fas蛋白的表达与其组织学分级、癌周侵袭程度及肿瘤包膜有关(1/3/7/1,4/12,1/4/7,P<0.05).而FasL蛋白的表达与其临床病理指标无明显关系(0/2/16/3,7/21,5/13/3,P>0.05).结论肝癌可能通过下调Fas、增强FasL的表达实现免疫逃避;而Fas的表达对辅助判断肝癌的侵袭转移乃至预后可能有重要意义。AIM To investigate the expression of Fas/FasL in hepatocellular carcinomas (HCC) and its correlation with clinicopathological characteristics. METHODS Sepecimens from 30 HCC cases were used for the evaluation of Fas/FasL expression by immunohi stochemistry and reverse-transcription polymerase chain reaction (RT-PCR). RESULTS Twelve cases (12/30, 40.0%) of HCC were detected to express Fas protein. The positive rate in HCC was lower than in pericancerous liver tissues. Twenty-one (70. 0%) csses of HCC had detectable FasL protein. There was no statistical significance as compared with pericancerous liver tissues (17/30 or 56.7% cases were positive for FasL protein). However, FasL mRNA level of HCC was higher than that of pericancerous liver tissues(70.1/32.2, p<0.05). In the same HCC, the expression of Fas/FasL mostly showed Fss^- FssL^+ (16/30, 53.3%). Fas expression was associated with histological differentiation, capsule and invaded pericancerous tissues (1/3/7/1, 4/12, 1/4/7, p<0.05), while no relationship was found between FasL expression and other clinical features (0/2/16/3, 7/21, 5/13/3, p>0.05). CONCLUSIONS HCC may eliminate or down-regulate Fas expression and up-regulate FasL expression to escape from the immune system. Fas expression was one of the important factors to determine the metastasis and the prognosis of HCC.
