摘　　要：目的　探讨腔隙性脑梗死 (lacunarinfarct,LI)患者血浆组织因子 (tissuefactor ,TF)和组织因子途径抑制物 (tissuefactorpathwayinhibitor,TFPI)测定的临床意义以及组织因子途径在LI发病中的作用。 方法　择确诊的LI患者 6 3例 ,采用酶联免疫吸附的方法测定血浆TF和TFPI相关指标抗原水平 ,与正常对照组比较并对不同危险因素患者组之间的结果进行分析。结果　①与正常对照组比较 ,LI患者组血浆TF抗原水平显著增高 (2 1 7.4± 1 0 1 .3pg/ml对 1 40 .9± 2 7.1 pg/ml,P =0 .0 0 0 3)、游离TFPI抗原水平降低 (41 .4± 1 6 .7ng/ml对 30 .0± 1 8.6ng/ml,P =0 .0 0 5) ;②合并高血压、糖尿病和血脂异常LI患者血浆TFPI相关指标的改变不同 ;③LI患者血浆t TFPI和tr TFPI抗原水平与血浆TF抗原水平相关。Objective To investigate the clinical significance of measuring plasma tissue factor (TF) and tissue factor pathway inhibitor (TFPI) antigen levels in patients with lacunar infarction (LI) and the effect of tissue factor coagulation pathway in the development of LI. Methods Sixty three patients with LI were selected into the study, the levels of plasma TF antigen and TFPI related index were measured by ELISA and compared with the normal control's, and then the TF and TFPI levels of known risk factor subgroups were analysed.Results Compared with normal control group ,TF antigen levels were elevated(217.4±101.3pg/ml vs 140.9±27.1pg/ml,p=0.0003) and free TFPI antigen levels were reduced (41.4±16.7 ng/ml vs 30.0±18.6 ng/ml,p=0.005)significantly;②the levels of plasma TFPI related index were different in LI subgroups united with hypertension ,diabetes mellitus and dislipidemia;③the levels of total TFPI and truncated TFPI antigen were associated with the levels of plasma TF antigen.Conclusion The tissue factor coagulation pathway had significant changes in patients with LI, the major manifestations were that the coagulation activity were elevated and the anticoagulation activity were decreased.
