摘 要:目的 探讨子宫颈癌发生中细胞凋亡与 p5 3基因蛋白表达及高危型人乳头样病毒 (HPV)感染的关系。方法 检测对象为正常宫颈鳞状上皮 (NE) 4 1例、重度非典型增生 (SD) 4 1例、原位癌 (CIS) 37例、早期浸润癌(MIC) 31例、大细胞非角化型浸润癌 (IC) 4 0例共计 190例手术切除福尔马林固定、石蜡包埋的组织标本。凋亡细胞的检出使用TDT -mediateddUTP -biotinnickendlabeling(TUNEL)方法 ,p5 3基因蛋白表达采用单克隆抗体免疫组织化学ABC染色方法 ,HPV16、18型E6DNA感染使用PCR方法进行检测。结果 TUNEL标记率从NE到CIS组呈现有意义地减少 (P <0 0 1) ;在SD、CIS组 ,p5 3基因蛋白超表达者TUNEL标记率呈现有意义的低值 (P <0 0 5 ) ;高危型HPV感染与宫颈癌的发生有关 ,与细胞凋亡、p5 3基因蛋白表达未呈现直接的相关性改变。结论 细胞凋亡参与宫颈癌发生的早期过程 ,其与 p5Objective To investigate the relationship between apoptosis with p53 protein and high risk human papillomavirus ( HPV ) infection in uterine cervical carcinogenesis. Method Samples of formalin-fixed, paraffin-embedded tissues were obtained from surgical resections in total of 190 cases, among whom 41 of normal cervical squamous epithelium ( NE ), 41 of severe dysplasia ( SD ), 37 of carcinoma in situ ( CIS ), 31 of microinvasive carcinoma ( MIC ) and 40 of frank invasive large cell non-keratinizing epidermoid carcinoma( IC ) . TDT-mediated dUTP-biotin nick end labeling ( TUNEL ) method and single-clone antibody immunohistochemical staining with ABC was respectively performed to detect the apoptotic cells and the expression of p53 protein. The infection of HPV type 16, 18 E6 DNA were determined by PCR. Results The TUNEL staining index was significantly decreased from NE to CIS ( P<0 01 ). In SD and CIS, the expression of p53 protein showed significantly correlative variation with the TUNEL staining index ( P<0 05). The infection of HPV type 16,18 DNA was associated with the carcinogenesis of the cervical carcinoma. But the correlative variation was found neither in apoptosis nor in the expression of p53 protein. Conclusion Apoptosis is associated with the early stage of the uterine cervical carcinogenesis and relates with the overexpression of p53 protein .
