血清GGT和GGT/ALT比值联合应用鉴别肝脏良恶性疾病

摘　　要：目的 探讨血清GGT活性及GGT／ALT比值在肝脏良、恶性疾病中的鉴别诊断价值。 方法 采用全自动生化仪分别检测了138例肝脏良、恶性疾病患者和60例无肝病其他肿瘤患者及132例健康体检者血清GGT活性及ALT活性。 结果 所有肝脏良恶性疾病组血清GGT均显著高于健康对照组（P＜0．01）和无肝病其他恶性肿瘤组（P＜0．01）；原发性肝癌组和肝转移癌组GGT活性均显著高于其他肝脏良性疾病组（P＜0．01或P＜0．05）。病毒性肝炎组GGT／ALT比值显著低于脂肪肝组、原发性肝癌组、肝转移癌组、肝硬化组、肝血管瘤组之间均有极显著差别（P＜0．01）；随着GGT阳性界值的提高，阳性率逐渐下降，但对肝脏恶性肿瘤诊断的特异性却显著提高。 结论 血清GGT活性和GGT／ALT比值联合应用可有效辅助鉴别肝脏良、恶性疾病。 Objective To evaluate the activity of serum gammaglutamyltransferase (GGT) and GGT／ALT (ALT Alanine aminotransferase) in differential diagnosis of benign and malignant hepatic diseases． Methods Serum GGT and ALT activities were determined in 30 patients with primary hepatic carcinoma (PHC) 29 with metastatic hepatic carcinoma (HMC) 8 with hepatic angioma (HA) 13 with fatty liver (FL) 13 with hepatic cirrhosis (HC) 46 with viral hepatitis (VH) 60 with carcinoma but no hepatic diseases and 132 healthy controls． The GGT／ALT ratios were also calculated． Results The results showed that serum GGT in patients with either benign lesion or malignant hepatic carcinoma was signifcantly higher than that in healthy control (P＜0．01) and patients with hepatic disease (P＜0．01) and GGT in patients with primary metastatic hepatic carcinoma was higher than that in those with benign hepatic diseases (P＜0．01 or P＜0．05) and GGT／ALT ratio in patients with viral hepatitis was significantly hower than that in those with malignant hepatic carcinoma fatty liver liver cirrhosis and hepatic angiometa (P＜0．01)． Together with elevation of positive cut－off value of GGT the positive rate lowered gradually but the specificity in diagnosing malignant tumor of liver was raised significantly． Conclusion Combined detection of serum GGT and GGT／ALT ratio is effectively helpful in differential diagnosis of benign and malignant hepatic diseases． 
