脑蛛网膜下腔出血患者血浆中NO和ET的动态观察及其临床意义

摘　　要：通过测定一氧化氮 (NO)和内皮素 (ET)在脑蛛网膜下腔出血 (SAH)患者血浆中含量的变化 ,探讨二者在SAH中的作用。应用放射免疫方法测定和观察 40例SAH患者急性期和恢复期血浆中NO、ET含量的变化并与2 0名正常人 (对照组 )进行比较。结果表明SAH急性期血浆中NO水平显著低于对照组 (P <0 .0 1) ,ET水平显著高于对照组 (P <0 .0 1) ;恢复期血浆中NO水平有所提高 ,高于急性期 (P <0 .0 1) ,但与对照组无统计学意义(P >0 .0 5 ) ,ET水平仍明显高于对照组 ,具有统计学意义 (P <0 .0 1) ,但略低于急性期 ,无统计学意义 (P >0 .0 5 )。NO和ET参与了SAH的病理生理过程 ,检测血浆NO和ET水平有助于病情的观察 ,正常情况下NO/ET比值处于动态平衡 ,急性期NO/ET比值下降 。To observe the dymamic changes in Endothelin(ET)and Nitric Oxide(NO) levels of plasma in patients with Subarachnoid Hemorrhage(SAH), the dynamic changes in plasma ET and NO in 40 patients with the acute SAH group, recovery SAH group and 20 normal group were measured by RIA. NO levels of plasma in the acute SAH group were significantly decreased compared to the normal group ( P <0.01), ET levels of plasma in the acute SAH group were significantly higher than that in the normal group( P <0.01); NO levels of plasma in the recovery SAH group were increased higher than that in acute group( P <0.01),but there was no statistically significant difference between the recovery SAH group and the normal group( P >0.05).ET levels of plasma in recovery SAH group were significantly higher than that in normal group( P <0.01),but there was no statistically significant difference between the acute group and recovery SAH group( P >0.05). This study shows that NO and ET could participate in the proces of pathology and physiology in SAH. Dynamic detection of NO and ET levels is helpful in analyzing SAH whether serious or mild. In the normal, NO/ET was the dynamic balance,NO/ET was decreased in the acute group,NO/ET was increased in the control SAH group.
