摘　　要：目的　观察动脉栓塞治疗子宫肌瘤的临床效果。方法　采用Seldinger方法动脉栓塞治疗 86例子宫肌瘤患者。经股动脉穿刺 ,将导管选择性插入双侧子宫动脉 ,经导管注入PVA和平阳霉素 ,阻断肌瘤血供。动脉栓塞 6个月后对患者的临床症状和超声检查结果进行分析 ,同时还分析了该方法的并发症和失败原因。结果　经血管造影发现 ,子宫肌瘤血供丰富 ,均由双侧子宫动脉供血 ,栓塞双侧子宫动脉后 ,子宫肌瘤血供可完全阻断 ,治疗后 6个月的随访结果表明 :月经量及月经周期恢复正常 ,疼痛明显改善 ,肌瘤和子宫明显缩小。临床有效率 91%。结论　动脉栓塞治疗子宫肌瘤是一种创伤小 。Objective To study the clinical effect embolization of uterine arteries in the treatment of uterine myomata.Methods 86 cases with symptomatic uterine myomata were treated by Seldinger's selective uterine arteries embolization.After a percutaneous femoral arterial puncture,an artery catheter was selectively inserted into bilateral uterine arteries.The polyvinyl alcohol(PVA)and /or pingyangmycin(PYM)were introduced through catheter to occlude the blood supply of myoma.Six months after the procedure,analysis was performed on ultrasound and clinical symptoms of patients.We also calculated complication and failure reason of this procedure.Results Arteriography showed that most uterine mymoata were supplied by bilateral uterine arteries,When both uterine arteries were embolized,the blood supply of uterine myoma could be occluded.With a mean follow-up of 6 months the menorrhagia and menstrual cycles returned to normal and,pain resolved.Conclusion Selective uterine artery embolization is a new method for treatment of uterine myomata with less trauma and have a good clinical results.
