摘  要:目的 :观察兔不完全性脑缺血时的脑血液动力学变化 ,探讨经颅多普勒 (TCD)在脑缺血动物模型检测中的应用价值及临床意义。方法 :新西兰兔 5 6只 ,分别结扎双侧颈总动脉及放血制成不完全性前脑缺血模型 ,使用 TCD于术前、术后立即、 6小时、 12小时、 2 4小时、 48小时检测双侧颈总动脉 (CCA)、双侧颈内动脉终末段(ICA)及基底动脉 (BA) ,记录血流频谱 ,计算各动脉的收缩期峰值流速 (Vp)、舒张期末流速 (Vd)、平均流速(Vm)及脉动指数 (PI)。结果 :术后所有动物 ICA血流信号消失 ,BA各时段的 Vp、 Vd、 Vm均较术前明显增快(P<0 .0 1) ,PI降低 (P<0 .0 1) ,部分动物 (2 3只 ,占 41.1% )可见湍流频谱并可闻及血管杂音。术后 48小时 BA的 Vp、、 Vd、 Vm均较术后立即检测有所下降 (P<0 .0 5 ) ,PI回升 (P<0 .0 1)湍流频谱及血管杂音减少。结论 :TCD可用来客观地观察和评价脑缺血兔的动物模型成功与否及侧支循环代偿情况 。Objective:To Observe the hemodynamics of incomplete cerebral ischemia in rabbits, and to find the value of transcranial Doppler(TCD) in the animal experiment.Methods:Fifty six New Zealand white rabbits were operated for bilateral common carotid artery occluding ligature and artificial hypotension to prepare a model of incomplete anterior cerebral ischemia,and examined with TCD on days before operation,immediately,6 hours,12 hours,24 hours and 48 hours after operation.The systolic peak velocity(Vp),diastolic velocity(Vd),mean velocity(Vm) and pulsatility index(PI) in bilateral common carotid atery(CCA),internal carotid atery(ICA) and basal atery(BA)were got from the cerebral blood spectra.Results: Blood signals of ICA disappeared in all animals with increased BA velocity and PI in all times after operation( P <0 01) Parts of animals(41 1%)revealed a confused blood spectrum and vessel bruits were heard At 48 hours after operation,Vp,Vd,Vm in BA were decreased( P <0 05),and PI were recovered ( P <0 01) ,Confused spectra and vessel bruits were lessened as compared with the results immediately after operation.Conclusions: Collateral circulation and effectiveness of rabbit cerebral ischemia can be evaluated with TCD ,which is helpful to find a new way of therapeutic effect judgment.
