立复丁(法莫替丁)治疗消化性溃疡并急性大出血疗效及抑酸效果的观察

摘　　要：目的　观察立复丁 (法莫替丁 )对消化性溃疡并急性大出血的治疗效果 ,并观察部分患者用药后胃内pH值的改变。方法　将消化性溃疡并急性大出血患者分为治疗组和对照组 ,治疗组 96例以立复丁 2 0mg ,静脉滴注 ,每天 2次 ;对照组 46例予西咪替丁 0 4g ,静脉推注 ,每天 3次。治疗组有 10例患者 ,于用药期间行 2 4h胃内pH值监测。结果　治疗组止血显效率为 72 92 % ,总有效率为 93 75 % ,无明显毒副作用。对照组止血显效率为 43 48% ,总有效率为 80 43% ,两组比较有显著性差异。 2 4h胃内pH值测定显示 2 4h内pH值 >3、pH值 >4和pH值 >6的时间比例分别为 5 7 0 5 %、43 97%和 2 1 5 5 % ,夜间胃内pH值明显升高。结论　立复丁治疗消化性溃疡并急性大出血效果良好 ,可使胃内pH值较长时间维持高值 ,特别对夜间pH值提高明显。Objectives To observe the clinical effects of Lifuding(Famotidine injection)on patients with acute peptic ulcer massive hemorrhage,and the changes of intragastric pH of the patients treated with Lifuding.Methods Patients with acute peptic ulcer massive hemorrhage were divided into Lifuding group and control group.Ninety-six patients in Lifuding group were given Lifuding 20 mg iv drip b.i.d.Fourty-six patients in control group were given Cemitidine injection 40 mg iv q 8 h.Twenty-four-hour intragastric pH monitoring was performed with subjects receiving Lifuding.Results The hemostatic effectual rate and global effective rate in Lifuding group and control group were 72 92% vs 43 48% and 93 75% vs 80 43% respectively.The differences were significant statistically.The results of 24-hour intragastric pH monitoring showed that the time ratio of pH>3,pH>4 and pH>6 were 57 05%,43 97% and 55% respectively within 24 hours,and nocturnal intragastric pH elevated markedly.Conclusions Lifuding(Famotidine injection)showed good effects on peptic ulcer hemorrhage.It can maintain intragastric pH at a high level during a long period of time,especially at night.
