摘　　要：目的 :评价B超引导经皮穿刺酒精硬化治疗单纯性肾囊肿的安全性、有效性和长期随访结果。方法 :我们从1988～ 2 0 0 0年对 32 0例单纯性肾囊肿病人进行B超引导经皮穿刺抽液注射酒精硬化治疗。大部分病例为门诊病人 ,在局部麻醉下进行操作。平均随访时间 4 8± 12个月 (6～ 16 8个月 )。随访间隔为治疗后第一年的第 3、6、12个月 ,B超复查囊肿大小 ,以后每 6～ 12个月复查一次。结果 :32 0例病人的经皮穿刺抽液注射酒精过程全部成功 ,所有囊肿平均直径从7 5 8± 2 .33cm(4.0～ 14 .0cm)降至 0 .5± 1.5cm(0 .5～ 2 .0cm) ,P <0 .0 0 0 1。 14 8例病人 (46 .1% )囊肿完全消失。在硬化治疗之前 ,2 78例病人 (86 .88% )有囊肿引起的不同症状 ,酒精硬化治疗后这些病人中的 2 6 1例 (93.88% )症状消失或减轻。无明显并发症。 3例复发 ,再次穿刺治疗有效。结论 :经皮穿刺抽液注射酒精硬化治疗单纯性肾囊肿方法简单、快捷、安全、有效、廉价。Objective:To evaluate the safety,efficacy,and long term outcome of percutaneous drainage and ethanol sclerotherapy under the ultrasonic guide for simple renal cysts.Methods:In a retrospective study,from 1988 to 2000,320 patients with a simple renal cyst were treated with ultrasound guided percutaneous aspiration,and sterile 99% ethanol was injected into the cyst.The procedure was performed under local anesthesia,and most of the patients were outpatients.The mean follow up period was 48±12(range 6 to 168) months.Control check up were scheduled at 3,6,and 12 months after the sclerotherapy,then at intervals of 6 or 12 months.Results:Sclerotherapy with ethanol was performed successfully in all 320 patients with a simple renal cyst.The cyst disappeared completely in 148 patients(46 1%).The mean size of all cysts decreased from 7.58±2.33(range 4 to 14)cm to 0.5±1.5(range 0.5 to 2.0)cm,P<0.0001.Before the sclerotherapy,278 patients(86.88%) had symptoms due to the cyst,and 261(93.88%) of these were asymptomatic or limitive affer the ethanol sclerotherapy.No major complications occurred.Conclusion:Ultrasound guided percutaneous aspiration and sclerotherapy with ethanol for simple renal cysts are simple,fast,safe,effective,and inexpensive.We propose sclerotherapy with 99% ethanol as the primary treatment of simple renal cyst.
