口服弥可保前后青光眼视野缺损的比较

摘　　要：目的　比较青光眼患者口服弥可保前后视野变化 ,评估弥可保对青光眼患者视野改善的作用。方法　首先 ,筛选有视野缺损、眼压控制在 2 1m m Hg(1k Pa=7.5 m m Hg)以下的 2 5例 49眼原发性开角型青光眼和 2 7例5 2眼原发性闭角型青光眼患者 ,口服弥可保 6个月 ,每日 3次 ,每次 5 0 0μg。然后 ,分别于用药 3、6个月用静态自动电脑Octpus 10 1型视野计进行视野检查。检查在半暗室下进行 ,检查程序采用 G2 (青光眼 ) 30度灰度阈值 5 9个点 ,背景光4asb;分别测试平均缺陷 (MD)、丢失方差 (L V)、刺激丢失方差 (s L V)、可靠性因素 (RF)及瞳孔变化。最后 ,记录结果并与用药前相比较。结果　 6 7.3%患者视野的平均缺损用药后 3、6个月有不同程度好转 ,6个月变化显著。用药前与用药后 3个月比较无显著性差异 (P >0 .0 5 ) ;与用药后 6个月比较有显著性差异 (P <0 .0 5 )。用药后 3、6个月比较也有显著性差异 (P <0 .0 5 )。6个月视野缺损的平均改善率开角型青光眼组与闭角型青光眼组分别为 5 1.6 5 %、44 .5 5 % ,2组间比较无统计学差异 (P >0 .0 5 )。结论　长期服用弥可保 ,对于眼压控制平稳的原发性开角型和原发性闭角型青光眼所致的视野缺损有明显改善。Objective To compare the changes of the visual field in primary open-angle and primary closed-angle glaucoma before and after oral Mecobalamin was taken and to evaluate its effects on the visual field.Methods First,25 cases(49 eyes) with the primary open-angle glaucoma and 27 cases(52 eyes) with primary closed-angle glaucoma,who had the visual field defect and whose IOP was≤21mmHg,were selected.Oral Mecobalamin 500μg( tid ),was taken for 6 months.Secondly,the visual field was strictly measured using a Octpus 101 before the treatment and in the third month and the sixth month after the treatment.The examinations were conducted in the semi-darkroom,mean defect(MD)?loss variance(LV)?stimulate LV(sLV)?reliability factor(RF) and pupil were tested respectively. Finally,the changes of the visual field before and after treatment were compared.Results The visual field defect of 67.3% patients was improved in different degrees in the third month and the sixth month after the treatment and the changes were great in the sixth month.There were no significant differences before the treatment and in the third month after the treatment(P>0.05),but there were significant differences in the sixth month after the treatment (P<0.05).There were also significant differences in the third month and the sixth month after the treatment(P<0.05).In the evaluation of visual field defects in the sixth month after the treatment,it was found that the average improvement rates in primary open-angle and primary closed-angle glaucoma respectively were 51.65% and 44.55%,there were no significant statistics differences between the two groups (P>0.05).Conclusion Long-term oral Mecobalamin administration is useful for the visual field defect improvement in patients with primary open-angle and primary closed-angle glaucoma with normal intraocular pressure.
