摘  要:目的 探讨巨大脑胶质瘤的分次γ刀治疗及其疗效。方法 巨大脑胶质瘤的γ刀治疗分两次或三次完成,每次间隔1d,分次照射的周边剂量为5~10 Gy。治疗效果通过MRI和临床表现综合评估。结果 随访3~33月,28例巨大脑胶质瘤分次γ刀治疗有效率为78.6%,2例病人γ刀术后出现迟发性脑水肿。治疗效果与肿瘤分化程度有关,而与肿瘤大小无关。结论 巨大脑胶质瘤分次γ刀治疗效果显著,但其远期疗效有待进一步的临床和实验观察。Objective To explore the treatment of giant gliomas with fractionated gamma knife surgery. Methods The two or three courses of low-dose treatment were planned for fractionated gamma knife surgery in 3 or 5 days with the marginal dose of 5~10Gy in each radiation session. The therapeutic results were evaluated on the MRI findings and clinical manifestations of these patients. Results After a follow-up of 3-33 months, the growth-controlled rate of 28 gliomas treated with fractionated gamma knife surgery was 78.6%, 2 cases showed delayed brain edema after the radiosurgery. The outcome of treatment was related to the extent of differentiation of the tumors and the times of the fractionated treatment, but not to the size of tumors. Conclusion The fractionated gamma knife surgery for giant gliomas is practicable, but further study and long-term follow-up are necessary to develop the fractionated gamma knife radiosurgery as a safe and effective therapeutic method for the malignant gliomas.
