摘  要:目的 比较异丙酚和依托咪酯全麻诱导时对口腔颌面部手术患者循环及儿茶酚胺的影响。方法  3 0例口腔颌面部择期全麻手术病人 ,随机分为两组 ,I组以异丙酚 2~ 2 .5mg/kg诱导 ,Ⅱ组以依托咪酯0 .2~0 .3mg/kg诱导 ,各 15例。观察诱导前 ,插管即刻 ,插管后 3min、10min血液动力学指标并抽取外周静脉血测定血浆肾上腺素 (E)、去甲肾上腺素 (NE)的水平。结果 异丙酚诱导气管插管能抑制高血压反应 ,不增加心肌耗氧量 ,明显优于依托咪酯 (P <0 .0 1) ;两药诱导气管插管均不能抑制E和NE升高 ,但Ⅱ组插管即刻、插管后 3minNE比I组升高显著 (P <0 .0 1)。结论 异丙酚对循环和儿茶酚胺的影响较小 。Objective To compare the effect of propofol and etomidate on circulatory and catecholamine to patients undergoing oral and maxillofacial surgery during induction. Methods Thirty adult patients ,scheduled for oral and maxillofacial surgery,were randomly allocated into two groups:I group(n 1 =15,induced with intravenous propofol 2.5 mg/kg);Ⅱ group (n 2 =15,etomidate 0.2 ~ 0.3 mg/kg). Hemodynamics was recorded, intravenous blood samples were taken before induction,during intubation and 3,10 min after intubation respectively. Results Propofol attenuated the increase in BP and the oxygen consumption of myocardium during induction and intubation( P < 0.01 ). Neither of the two drugs inhibited increase in E and NE during induction and intubation, but in group Ⅱ NE was markedly higher than that in group Ⅰ( P < 0.01 ). Conclusion Propofol has less effects on circulatory and catecholamine ,and is an intravenous anesthetic of choice for induction.
