摘  要:目的 探讨P gp、TopoⅡ和GST π在大肠癌的表达情况及其临床意义。方法 运用免疫组织化学技术 (SP法 )检测P gp、TopoⅡ和GST π在 6 9例大肠癌中的表达情况。 结果  (1) 6 9例大肠癌中 ,P gp表达者 6 0 87% ,TopoⅡ表达者 5 9 42 % ,GST π表达者 79 71% ,P gp和GST π共同表达者 5 2 17%。 (2 )P gp在高分化腺癌中高表达 ,在低分化腺癌低表达 (P =0 0 2 6 )。 (3)单因素分析表明 ,P gp表达与大肠癌患者的生存期和不良预后有关。Cox回归分析表明 ,P gp表达、P gp和GST π共同表达与患者的生存率有关 ,TopoⅡ表达对患者有保护趋势。结论 实验表明 ,P gp、TopoⅡ和GST π等多因素联合作用是大肠癌多药耐药性的主要作用机制。对大肠癌患者进行此三项指标的测定 。Objective To elucidate the relationship of the expression of P-gp,TopoⅡ α and GST-π in colorectal carcinomas and their clinical significance.Methods The expression of P-gp,TopoⅡ α and GST-π in 69 cases of colorectal carcinomas were investigated using immunohistochemical SP method.Results (1)Of the 69 cases of colorectal carcinomas,62.32% were positive for P-gp,59.42% for TopoⅡα,and 79.71% for GST-π.The co-expression of P-gp and GST-π was 52.17%.(2)A significant relationship was found between P-gp expression and pathological grade,that is,the cases with high expression showed good differentiation and those with low expression showed poor differentiation (p=0.026).(3)The analysis of single factor revealed a significant relationship between P-gp expression and the prognosis of patients with positive P-gp expression had a shorter average survival time.The correlation with prognosis was analyzed by Cox proportion hazard model.It showed that P-gp expression and co-expression of P-gp and GST-π were independently associated with patient survival length (P<0.05),while TopoⅡα may play a protective role.Conclusions The experiment shows that the combinative effect of P-gp,TopoⅡα and GST-π may be the chief mechanism of multidrug resistance of the patients with colorectal carcinoma and P-gp plays the most important role.The assay of P-gp,TopoⅡα and GST-π in patients with colorectal carcinoma is important and has instructive significance to estimate prognosis and work out a chemotherapy plan.
