摘  要:目的 研究两型 5α 还原酶同工酶在人前列腺癌组织中的表达情况及其意义。方法 应用逆转录 聚合酶链反应 (RT PCR)及免疫组化方法 ,对 15例正常前列腺组织 (取自尸体肾移植供体及全膀胱切除标本 )及 15例前列腺癌组织中两型 5α 还原酶的表达进行了定性和半定量研究。结果 正常及前列腺癌组织中都存在着两型 5α 还原酶mRNA及其酶蛋白的表达。Ⅰ型还原酶在腺癌组织中的表达明显高于正常组织 ,前列腺癌组织中Ⅱ型还原酶的表达相对较弱。Ⅰ型还原酶在前列腺癌组织中的表达水平与肿瘤的分期、分级及血清PSA水平存在正相关。结论 Ⅰ型 5α 还原酶在前列腺癌的发生、发展过程中起着重要作用 ,Ⅰ型 5α 还原酶抑制剂或两型 5α 还原酶的双重抑制剂可应用于前列腺癌的治疗。Objective To study the relative tissue distribution and expression pattern of type Ⅰ and type Ⅱ 5α reductase isozymes in prostate cancer tissues. Methods Immunohistochemistry and RT PCR method were used to investigate qualitatively and semi quantitatively the expression of type Ⅰ and type Ⅱ 5α reductases in prostate tissues from 15 normal persons and 15 patients with prostate cancer. Results Two isozymes were detected in both normal and cancerous prostate tissues with the two methods. Both 5α reductases were mainly localized in the cytoplasm, and higher degree of staining was observed in epithelial cells than in stroma. Significantly higher level of type Ⅰ 5α reduetase expression was observed in cancerous tissue than in normal tissue. The expression level of type Ⅰ 5α reduetase was positively correlated with the tumor stage and grade, and serum PSA concentration. The expression of type Ⅱ 5α reductase was very weak in prostate cancer tissue. Conclusion The type Ⅰ 5α reductase, not type Ⅱ, is involved in the pathogenesis of protate cancer. Selective type Ⅰ 5α reductase inhibitors or dual inhibitors of both type human 5α reductase isoforms can be used in the treatment of prostate cancer.
