摘  要:目的 探讨颅脑创伤后脑水肿的变化 ,并寻找在体研究脑水肿的方法。方法 气体冲击致大鼠中度颅脑创伤后 ,以干湿重法和多频率阻抗法监测伤后脑水肿的变化。结果 伤后 3小时 ,伤区脑组织已明显水肿 ,伤后 2 4~ 72小时 ,伤区脑水肿达高峰 ,伤后 7天基本恢复。多频率阻抗测试表明 ,伤后早期细胞外液电阻 (Re)及其与细胞内液电阻 (Ri)的比Re/Ri明显下降 ,提示发生了细胞外水肿 ;伤后 6小时后Ri明显下降 ,Re/Ri明显增加 ,提示细胞内水肿的发生。结论 颅脑创伤后早期发生血管源性水肿 ,而随之发生细胞毒性水肿 ,两种水肿的发展使脑水肿于伤后 2 4~ 72小时达高峰。多频率阻抗测试可在体监测脑水肿的变化。Objective To explore the changes of brain edema after traumatic brain injury, and to find the method applied to study brain edema in vivo. Methods The moderate brain injury of rats was produced by gas percussion. The changes of brain edema were measured with wet dry weight method and measurement of impedance at many frequencies. Results Distinct brain edema of injured brain tissue was found at 3 hour after trauma. Brain edema went forward the most severe at 24~72 hour after trauma, and recovered at 7 day after trauma. Measurement of impedance at many frequencies found that the resistance of extracellular fluid(Re) and Re/Ri(Ri, resistance of intracellular fluid) significantly decreased in early stage after trauma, which indicated the appearance of extracellular edema. Six hours later Ri decreased markedly, and Re/Ri elevated significantly, which indicated the occurrence of intracellular edema. Conclusions Vasogenic brain edema was occurrde at the early stage of TBI, and then cytotoxic brain edema followed. The development of both types of edema made that brain edema reached the height at 24~72 hour after trauma. The measurement of impedance at many frequencies might show the development of traumatic brain edema in vivo.
