彩色多普勒能量图在鉴别诊断乳腺良恶性肿块中的临床价值

摘　　要：目的　探讨能量多普勒超声 (CDE)在鉴别诊断乳腺良恶性肿块中的临床价值。方法　应用能量多普勒技术检测 90例乳腺良恶性肿块 ,并与彩色多普勒血流图 (CDFI)技术相比较 ,分析肿块的血流检出率、分布、形态及血流丰富程度。结果　乳腺良性肿块的CDE血流检出率为 95 % ,乳腺恶性肿块为 1 0 0 % ,与CDFI相比均有提高。 87.5 %乳癌的CDE显示有Ⅱ、Ⅲ级血流 ,其中 ,5 5 %乳癌的CDE血流图表现为粗细不一、扭曲状的血管网。结论　CDE技术提高了乳腺肿块血流的检出率 ,并能生动、完整地显示乳腺癌的丰富血管网 。Objective To explore the clinical value of color Doppler energy (CDE) in differential diagnosis of benign and malignant breast masses. Methods CDE was used to detect 90 patients with breast neoplasm after pathological confirmation, and the results were compared with color Doppler flow imaging (CDFI) on detection rate of blood flow, pattern of vascularity, and morphology of vessels. Results The rate of flow detected by CDE was 95% in benign breast neoplasm, and 100% in malignant tumor, both higher than those by CDFI. About 87.5% malignant breast neoplasm showed Ⅱ and Ⅲ grade vascularity on CDE imaging and 55% manifested irregular and contorted vascular network. Conclusion CDE enhances the detection rate of blood flow and shows the vascular net of malignant breast neoplasm vividly and completely, and may be regarded as a valuable method for diagnosing malignant breast neoplasm.
