食管癌组织上皮型钙粘附素及β-链接素表达和预后关系——附106例分析

摘　　要：目的 :旨在探讨上皮型钙粘附素 (E cd)及 β 链接素 (β cat)表达是否与食管鳞状细胞癌病人预后有关。方法 :通过免疫组化S P法检测 10 6例食管癌组织E cd及β cat的表达。结果 :E cd和 β cat表达降低及缺失率分别为 66.0 % (70 /10 6) ,69.8% (74 /10 6)。E cd表达强度与组织学分级 ,肿瘤大小淋巴转移 ,临床分期和静脉侵犯呈负相关 ,表达正常者 1、3、5年生存率均高于低表达者 (P =0 .0 5 32 ) ,尤其在Ⅰ、ⅡA、ⅡB期 (P =0 .0 0 12 )。β cat表达强度与组织学分级呈负相关 (P =0 .0 14) ,表达缺失者生存时间短于正常表达者 (P =0 .0 5 0 1)。E cd和 β cat表达程度之间存在正相关 (P =0 .0 0 5 )。Cox比例风险模型分析指出E cd表达下调是显示预后不良的重要因素。结论 :E cd作为独立的预后因子 ,其表达与总体生存时间和无病生存时间有关 ,可判断食管癌病人的预后 ,而 βObjective To elucidate whether the expression of E cadherin(E cd)and β catenin(β cat)correlate with clinical outcome in patients with oesophageal squamous cell carcinoma(OSCC).Methods Expression of E cd and β cat were determined by S P imunohistochemical technique in 106 consecutive patients with OSCC.Results The loss and reduction rate of E cd and β cat expression was 66.0%(70 cases)and 69 8%(74 cases),respectively.The intensity of the E cd expression had a negative correlation with histological grade,tumor size,lymph node metastasis,clinical stage and venous invasion.Patients with normally expressing tumors had a better prognosis at 1,3,5 years than those with low expressing and no expressing tumors( P =0.053 2),especially in Stage Ⅰ,ⅡA,ⅡB( P =0.001 2),There obviously existed inverse correlation between intensity of β cat expression with differentiation grade( P =0.014),and patients with no expressing tumors had shorter survival time than those with normally expressing tumors( P =0.050 1).Furthermore,Correlation analysis showed class of E cd positively correlated with that of β cat( P =0.005).Cox proportional hazard analysis suggested downregulation of E cd is an important factor that indicates poor prognosis.Conclusions As an independent prognostic factor,which correlated with overall and disease free survival time,expression of E cd but not β cat may predict prognosis in patients with OSCC.
