摘　　要：目的　探讨胃平滑肌肿瘤的临床特点及诊断、治疗方法 ,以期提高该病的早期诊断和治疗。方法　回顾性分析我院 1988～ 1999年收治的 78例胃平滑肌肿瘤 ,均经手术病理证实。结果　 78例胃平滑肌肿瘤中肌瘤 36例 ,肉瘤 42例。临床表现为消化道出血 (6 4.1% )、腹块和腹痛。X线钡餐和胃镜检查为重要诊断手段 ,但术前诊断率仍较低 (30 .8% )。本组行局部切除术 46例 ,胃大部切除术 32例。无术后并发症及手术死亡。结论　胃平滑肌肿瘤常表现为消化道出血、腹块、腹痛 ,当临床出现上述症状时 ,应高度怀疑本病 ,并及时行钡餐和胃镜检查 ,以期早期诊断。手术切除为本病唯一有效的治疗 。Objective To investigate the clinical characteristics,diagnosis and treatment of smooth muscle tumors of the stomach.Methods Seventy eight cases of smooth muscle tumors of the stomach treated in this hospital from 1988 to 1999 were analyzed retrospectively.Results This group included 36 cases of leiomyoma and 42 cases of leiomyosarcoma;all were histopathologically proven cases.Their clinical manifestations were gastrointestinal hemorrhage(64.1%),adbominal mass and pain.The diagnoses were established mainly on the basis of barium meal and fibrogastroscopy,but the accuracy rate of these measures was only 30.8% in this series.46 patients were subjected to local resection,and the others were given subtotal gastrectomy.There was no post operative complication or death from operation.Conclusion The clinical manifestations of smooth muscle tumors of the stomach comprised gastrointestinal hemorrhage,abdominal mass and pain.Barium meal and fibrogastroscopy should be done,if necessary.Apparently,surgical removal of the tumor is the best choice with good prognosis.
