摘 要:目的 对趋势导向蓝黄视野检查(tendency-oriented blue-on-yellow perimetry, TOP/BYP)、常规蓝黄视野检查(normal blue-on-yellow perimetry, N/BYP)及常规白白视野检查(white-on-white perimetry, WWP)结果进行比较,评价TOP/BYP在青光眼视功能检测中的应用价值。方法 利用OCTOPUS 101全自动视野计分别对19例(38眼)正常人和27例(54眼)原发性开角型青光眼(primary open-angle glaucoma, POAG)分别进行TOP/BYP、N/BYP及WWP检查,将三种视野检查结果、视野检查指数及视野缺损点数进行比较和分析。结果 TOP/BYP检查的特异性与N/BYP检查的特异性一致(89.47%);对已确诊的早期POAG,TOP/BYP检查的阳性率为86.49%,明显高于WWP的阳性率(54.05%),且与N/BYP检查阳性率(89.19%)之间无显著性差异;对中期和晚期POAG,TOP/BYP检查与N/BYP和WWP检查的符合率达100%;TOP/BYP和N/BYP检查POAG视野缺损点数明显大于WWP视野缺损点数。绪论TOP/BYP检查青光眼视功能损害的敏感性和特异性与N/BYP相当,TOP/BYP检测时间短,患者不易疲劳,是理想的蓝黄视野检查方法之一。Objective To compare the results obtained by tendency-oriented blue-on-yellow perimetry (TOP/BYP), normal blue-on-yellow perimetry (N/BYP) and normal white-on-white perimetry (WWP), and evaluate of TOP/BYP in detecting the visual function of glaucomas. Methods The TOP/BYP, N/BYP and WWP in the OCTOPUS 101 perimetry were used to examine the visual fields of 19 normal subjects (38 eyes) and 27 cases (54 eyes) of primary open-angle glaucoma (POAG),respectively. The visual field outcomes, global indices and defective points of the three perimetries were compared and analyzed. Results The specificity (89.47%) of TOP/BYP was coherent with that of N/BYP. The positive rate of TOP/BYP for the early POAG was 86.49%, which was higher than that of WWP (54.05%), but no significant difference was found between the positive ratesobtained by TOP/BYP and N/BYP. The positive rate of TOP/BYP (100%) for the middle and late stage POAG was coherent with that of N/BYP and WWP. The average numbers of defective points in central visual field detected with TOP/BYP and N/BYP were more than that with WWP in POAG. Conclusion The sensitivity and specificity of TOP/BYP in detecting the visual function of glaucoma is coherent with that of N/BYP. The checking time of TOP/BYP is short, and the patient will not feel fatigue easily, so it was one of the ideal method for blue-on-yellow perimetry.
