玻璃体切割联合FLURON填充治疗复杂性视网膜脱离9例

摘　　要：目的 探讨玻璃体切割术联合FLURON注入治疗难治性、复杂性视网膜脱离的方法、疗效、并发症及与惰性气体、硅油注入的比较。方法 分析9例9眼难治性、复杂性视网膜脱离,其中外伤所致的复杂性视网膜脱离3例;视网膜脱离复位术後再脱2例;巨大裂孔4例。病例中合并黄斑裂孔5例。行三切口经睫状体平坦部玻璃体切除术、FLURON(F_6H_8)填充术。术中6例环扎+外垫压+冷凝,4例行黄斑裂孔边缘激光光凝或巨大裂孔边缘激光光凝,2例行内界膜剥出,2例行晶状体咬切术。结果 术後患者视力均优于术前,网膜均平复,有2例出现一过性高眼压,2例出现白内障(前囊和後囊後混浊)。结论 玻璃体切割联合FLURON注入治疗难治性、复杂性视网膜脱离有良好的疗效,与惰性气体、硅油注入比较对下方及後极部的裂孔有更好的疗效;更方便的操作;病人更舒适的体位的优势。但有引起白内障和继发性青光眼的可能。Objective To study the technique, effect, complications of FLURON filling combined with vitrectomy in treatment of complicated retinal detachment and to compare the treatment with fillings of inert gases or silicone oil. Methods Retrospective study was made of 9 cases of refractory, complicated retinal detachment, treated with vitrectomy and FLURON filling. Among them, 3 cases were traumatic, complicated detachment, 2 were the second detachment after the previous retinal attachment and 4 were giant retinal tear. Results In all cases, the vision was improved and the retina even. Transient high intraocular pressure developed in 2 eyes and cataract (anterior and posterior ) in 2 eyes. Conclusion The combined treatment is very effective. Compared with fillings with inert gases or silicone oil, it is more effective for tears in the inferior retina or at the posterior pole; it is more convenient; the patients' posture is more comfortable. However, cataract and glaucoma can occur as complications.
