联合手术治疗虹膜根部断离的临床观察

摘　　要：目的 :探讨联合手术治疗虹膜根部断离 (iridodialysis)的疗效。方法 :2 5只眼患者在虹膜根部断离复位术中同时联合囊外白内障摘除及人工晶状体植入术 11只眼 ;联合晶状体玻璃体切割术 11只眼 ;联合玻璃体切割术 2只眼 ;联合睫状体复位术 1只眼。结果 :联合术后视力提高者 2 2只眼 ,视力不变者 2只眼 ;术前继发青光眼 9只眼 ,术后眼压控制者 7只眼 ,经二次抗青光眼手术后眼压控制者 2只眼。虹膜根部断离均复位满意。结论 :联合手术治疗虹膜根部断离不仅使患者尽早解除单眼复视 ,改善外观 ,而且使大部分患者只通过一次手术达到了脱残的目的 。Objective:To evaluate the effect of combined surgery in the treatment of iridodialysis.Methods:Reduction of iridodialysis was performed in 25 eyes (25 patients).11 eyes of these patients were combined with extracapsular extraction of cataract and IOL implantation,11 with phaco vitrectomy,2 with vitrectomy and 1 with ciliary body reduction.Results:The postoperative visual acuities were increased in 22 eyes,and the same as preoperative visions in 3 eyes.The postoperative intraocular pressures were controlled in 7 of 9 patients with secondary glaucoma,the others were re performed with anti glaucoma operation.In all patients the effects of iridodialysis reduction were satisfactory.Conclution:Combined operation treating iridodialysis not only avoids the occurrence of monocular diplopia,improves cosmesis,but also helps patients to get rid of blindness and restore stereoscopic visual acuity by operation at one time.
