转化生长因子TGF-α和P53基因在胆囊癌中的表达及临床意义

摘　　要：目的 :探讨TGF α和P5 3表达与胆囊癌临床病理特征及预后的关系。方法 :采用免疫组化法检测 4 3例胆囊癌和 10例胆囊腺瘤及 5例正常胆囊组织TGF α、P5 3表达。结果 :TGF α在正常胆囊组织中表达阴性 ,在腺瘤中表达阳性率为 2 0 % ,在腺癌中为 37 2 % ,三者之间差异无统计学意义 (P >0 0 5 )。P5 3蛋白在正常胆囊组织和腺瘤中表达均阴性 ,在腺癌中表达阳性率为 5 1 2 % ,后者明显高于前者 (P <0 0 5 )。TGF α、P5 3表达与胆囊癌病人年龄、性别是否合并胆囊结石无关 ,而与淋巴结转移有关 (P <0 0 5或P <0 0 1)。胆囊癌 1 5年生存率在P5 3和TGF α同时阳性表达者为 0 ,单一阳性表达者为 4 6 7% ,两者均阴性者为 92 3% ,三者比较差异有统计学意义。结论 :检测胆囊癌组织TGF α和P5Objective:To evaluate the relationship between the expression of TGF α,P53 and clinicopathological characteristics as well as prognosis in human gallbladder carcinoma.Methods:Immunohistochemistry was used to examine the expression of TGF α and P53 in 43 cases of gallbladder carcinoma and 10 cases of gallbladder adenomas and 5 speciments of normal gallbladder tissues.Results:The incidences of TGF α positivity in adenocarcinomas,adenomas and normal tissues of gallbladder were 37.2%,20% and zero respectively,and the difference was not statistically significant( P>0.05). No positive expression of P53 was found in gallbladder adenomas and normal tissues,while the positive rate of P53 in adenocarcinomas was 51.2%,the later was significantly higher than the former( P <0.05).The expression of TGF α,P53 alone was not correlated with patients′age,sex and the prevalence of gallbladder stone.However,a significant correlation was found between their expression and lymphnode metatasis.The 1.5 year survival rate in the group of concomitant positivity was zero,which was statistically lower than that in the group of only P53 and TGF α was positive or that in the concomitantly negative group.Conclusion:The expression of TGF α and P53 is a useful tool for providing information about the tumor malignancy and the prognosis of patients with gallbladder carcinoma.
