分开切口小梁切除联合白内障超声乳化人工晶体植入术  

Trabeculectomy with Apart Incision Combined with Phacoemulsification Intraocular Lens Implantation for Cataract Patients with Glaucoma

摘　　要：目的 :探讨表麻下分开切口小梁切除联合白内障超声乳化人工晶体植入术 (三联术 )的安全性和有效性。方法 :在表麻下 ,分开切口对 30例 (30只眼 )白内障合并青光眼患者行三联术。结果 :术前平均眼压 :2 6 .2 8mmHg ,术后随访平均眼压降至 (13.83± 1.2 8)mmHg ,术后矫正视力≥ 0 .5者 2 5例 (占 83.3% ) ,术后早期并发症 :角膜水肿10只眼 (33% ) ,浅前房 2只眼 (15 % ) ,脉络膜脱离 1只眼 (3% )。结论 :在表麻下分开切口行三联手术治疗青光眼合并白内障患者 ,具有视功能恢复快 ,降压效果稳定 。Objective: To evaluate the therapeutic efficacy and safety of Phacoemulsification intraocular lens implantation and trabeculectomy (triple procedure) combined with apart incisions under topical anesthesia. Methods:Thirty patients (30 eyes) with coexisting glaucoma and cataract underwent apart incision triple procedure under topical anesthesia. Results:The mean preoperative intraocular pressure (IOP) was 26.28 mmHg and postoperative IOP was 13.83±1.28 mmHg. 83% of 30 eyes had a best corrected visual acuity of ≥ 0.5.The early postoperative complications included corneal edema in 10 eyes (33%), shallow anterior chamber in 2 eyes (15%), and choroid detachment in 1 eye(3%).Conclusion:Under topical anesthesia, the apart incision triple procedure have advantages of improving acuity function rapidly, reducing IOP,and having few postoperative complications in treatment of patients with coexisting cataract and glaucoma.
