摘 要:目的 观察钌红染色方法对博莱霉素 (BL M)肺损伤早期肺表面活性物质 (PS)的超微结构的效果 .方法 BL M气管内滴注制作大鼠肺损伤模型 ,分为 3,7,14和 2 8d组 ,用钌红行肺组织标本染色 ,观察 PS的改变与肺泡 型上皮细胞 (AT- )结构变化间的关系 .结果 注药后各组均可见 PS层丧失连续、均匀绒状结构 ,脱落入肺泡腔或聚集成块或散落于肺泡腔内 ,以 3d组较为明显 .钌红阳性表面层的厚度与对照组比较 ,3d组较厚 (P<0 .0 1) ,且染色深 ,7及 14d组无明显差别 ,2 8d组较淡薄 (P <0 .0 1) .相应的 3及 7d组可见AT- 变性坏死 ,甚至崩解 ,以 3d组较明显 .各组均可见AT- 增生 ,以 7d组较明显 .板层小体 3d组数量减少 ,并呈空泡状 .7d组开始增多 ,以 14及 2 8d组较为明显 .14d组可见有 AT- 转化为 AT- ,并逐渐伸展、粘附于裸露基底膜 .结论 钌红染色法可敏感显示 BL M损伤 AT- 后 PS的形态特征而直接反映 AT-AIM To investigate the ruthenium red (RR) staining on the visualizing of ultrastructures of pulmonary surfactant (PS) in injured lungs caused by bleomycin (BLM). METHODS The lung injury model was conducted by intratracheal instilling of BLM, and all rats were divided into four groups: 3, 7, 14 and 28 d respectively. The alteration of PS and its relationship with the structural change of alveolar type II cells (AT Ⅱ) were observed after the lung samples were stained histochemically and examined by electron microscope. RESULTS After the treatment, it was observed in all groups that the homogenous and chorionic structure of PS were broken, with PS dropping into pulmonary alveolies, either aggregating or scattering. This injury was more obvious in 3 d group. Compared with the control group, RR positive surface was thicker in 3 d group ( P < 0.01 ) but thinner in 28 d group ( P <0.01), while no difference in 7 d group and 14 d group. It could also be found the degeneration and necroses of AT Ⅱ in both 3 d and 7 d groups, but 3 d group was more obvious. Whereas the proliferation of AT Ⅱ could be observed in all groups, and more obvious in 7 d group than in other groups. The lamellar body was found to decrease in 3 d group, and started to increase in 7 d group, while became obvious in 14 d and 28 d groups. Moreover, it could be found in 14 d group that AT Ⅱ transformed to AT Ⅰ, expanded gradually and attached to the bare basement membrane. CONCLUSION Ruthenium red staining is a sensitive method to visualize the structure features of PS after the AT Ⅱ was injured by BLM, therefore it could directly show the AT Ⅱ proliferation activity.
