小梁切除术联合应用丝裂霉素C及巩膜瓣可拆除缝线治疗青光眼的效果观察

摘　　要：目的 :探讨小梁切除术联合应用丝裂霉素C(MMC)及巩膜瓣可拆除缝线对青光眼的治疗效果。方法 :对 43例 6 0眼各类青光眼随机分为A组 2 0例 30眼和B组 2 3例 30眼 ,A组采用小梁切除术联合应用MMC及巩膜瓣可拆除缝线 ,B组行传统小梁切除术。结果 :术后随访 7～ 2 5个月。手术成功率A组 (93 3% )明显高于B组 (6 0 % ,P <0 0 1) ;功能性滤过泡A组(93 3% )亦明显高于B组 (5 3% ,P <0 0 1) ;浅前房A组 (3 3% )则低于B组 (2 6 % ,P <0 0 5 ) ;低眼压等并发症两组对比无显著性差异。结论 :小梁切除术联合应用MMC及巩膜瓣可拆除缝线在提高成功率的同时减少了并发症的发生 。Objective:To analyse the effect of trabeculectomy combined with the use of adjunctive mitomycin C and releasable suture of the scleral flap in treating glaucoma.Methods:60 eyes of 43 cases with various glaucoma were randomly distributed into tow groups:group A and group B.The patients were treated by trabeculectomy combined with the use of adjunctive mitomycin C and releasable suture of the scleral flap in group A and by routine trabeculectomy in group B.Results:Follow up 7 to 25 months, the rate of surgical success in group A(93 3%)was higher than that in group B(60%,P<0 01),the rate of functional filtering bleds in group A(93 3%)was higher than that in group B(53%,P<0 01)too.The rate of shallow anterior chamber in group A(3 3%)was lower than that in group B(26%,P<0 05).However,other complications such as hypotony were no statistically significant differences between the two groups.Conclusions:This method increased the safety and success ratio of the operation and decreased complications,it could be applied as a new resistant glaucoma operation pattern.
