现代白内障囊外摘除术后囊破裂术中处理的临床观察

摘　　要：目的　讨论白内障囊外摘除术后囊破裂的术中处理方式及效果。方法　对 37例(37只眼 )白内障囊外摘除术后囊破裂的病例进行回顾性分析。结果　后囊中央裂口 <5mm ,周边缺损 <12 0 者 18只眼 ;后囊缺损而前囊膜周边完整者 8只眼 ,常规法植入人工晶体 ,术后反应轻微。后囊破裂超过上述范围 11只眼 ,玻璃体处理后行双侧晶体襻悬吊术 ,术后反应重。术后裸眼视力 0 .1～ 0 .3者 5只眼 ,0 .3～ 0 .5者 4只眼 ,>0 .5者 2 8只眼。结论　根据后囊破裂的程度和部位 ,采取正确的处理方式 ,可以取得较好的临床效果。Objective To investigate the management and effect of extracapsular cataractopiesis with posterior capsular rupture.Methods 37 patients (37 eyes)of extracapsular cataractopiesis with posterior capsular rupture were reviwed retrospectively.Results 18 eyes that central posterior capsular rupture was <5mm and peripherial capsular rupture was <120 and 8 eyes that posterior capsule was fracted but periphery of anterior capsule was complete,were performed intraocular lens implantation routinely.Post operative reaction was slight.11 eyes that posterior capsular rupture were larger than that were performed intraocular lens implantation with both haptics fixation after vitreous management.Post operative reaction was serious.After operation nude visual acuity was 0.1～0.3 in 5 eyes,0.3～0.5 in 4 eyes and >0.5 in 28 eyes.Conclusion According to the position and size of posterior capsular rupture,correct management may achieve good clinical effect.
