摘  要:目的 研究LewisX(LeX) ,唾液酸化LewisX(sialyl-LeX) ,LewisA(LeA)和唾液酸化LewisA(sialyl-LeA)在大肠癌中的表达状况及其与肿瘤发生、分化、转移及预后的关系。方法 应用免疫组化微波 -LSAB法检测 90例大肠癌、51例转移淋巴结和 30例远离癌组织的正常大肠粘膜LeX ,sialyl-LeX ,LeA和sialyl-LeA的表达状况 ,并对其中 53例患者进行随访。 结果 LeX ,sialyl-LeX ,LeA和sialyl-LeA阳性物质在远离癌组织的正常大肠粘膜中阳性表达率分别为 90 .0 % (2 7/ 30 ) ,1 6 .7% (5/ 30 ) ,86 .7% (2 6/ 30 )和 80 .0 % (2 4 / 30 ) ,而在大肠癌组织中阳性率分别为 83 .3 % (75/ 90 ) ,92 .2 % (83/ 90 ) ,77.8% (70 / 90 )和 77.8% (70 / 90 ) ,转移淋巴结标本中阳性率分别为 66 .7% (34/ 51 ) ,94.1 % (48/ 51 ) ,62 .7% (32 / 51 )和 64 .7% (33/ 51 )。结论 癌细胞膜Lewis糖抗原的唾液酸化及异常糖基的形成 ,可能与肿瘤转移行为有关。Sialyl-LeX可以作为预测大肠癌侵袭、转移的肿瘤相关抗原 ,对评估患者预后较LeX ,LeA和sialyl-LeA具有更重要的临床意义。Objective To investigate the levels of cell adhesion molecule LeX, sialyl-LeX, LeA and sialyl-LeA expression in colorectal metastasis and prognosis. Methods The immunohistochemical expressions of sialylated and non- sialylated forms of LeX and LeA were studied in 90 carcinomas and 30 normal mucosal specimens of colon and rectum, as well as in 51 metastatic lymph nodes, by using microwave-LSAB immunohistochemical method. 53 of the patients were followed up. Results Their antigens were expressed in normal mucosa deviated from carcinoma lesions with the following rates: LeX 90% (27/30), sialyl-LeX 16.7% (5/30), LeA 86.7% (26/30), and sialyl-LeA 80.0% (24/30), whereas in carcinomas, the correspondent rates were as follows: LeX 83.3% (75/90), sialyl-LeX 92.2% (83/90), LeA 77.8% (70/90), and sialyl-LeA 77.8% (70/90). In metastatic lesions of lymph node, LeX, sialyl-LeX, LeA and sialyl-LeA were expressed in 66.7% (34/51), 94.1% (48/51), 62.7% (32/51) and 64.7% (33/51) respectively. Conclusion The alteration of Lewis-related carbohydrate antigens in cancer cell membranes, including sialylation and aberrant glycosylation, may be ralated to metastatic behavior. This suggests that dislyl-LeX may be used for estimating infiltration, metastasis and prognosis as a tumor-associated antigen in colorectal carcinoma.
