摘  要:摘 要:目的 探讨脑梗死患者血浆凝血酶原片段F1+ 2、D- 二聚体浓度的变化规律及丹参抗凝的临床意义。方法 用酶联免疫吸附法(ELISA)检测脑梗死患者血浆F1+2、D二聚体浓度,观察其在丹参干预治疗前后的变化,并对其中6例进行动态观测。结果(1)脑梗死患者血浆F1+2、D-二聚体浓度明显高于正常对照组(P<O.01)。(2)血浆F1+2 浓度在发病时显著高于正常,丹参治疗川后降至正常;D-二聚体发病时增高,丹参治疗21d后降至正常水平。(3)多发性或大面积梗死患者血浆F1+2、D-二聚体浓度明显高于腔隙性梗死患者(P<0.01)。(4)脑梗死患者24h内血浆F1+2、D-二聚体浓度异常率均高于头部CT阳性率(P〈0.01)。结论(1)脑梗死患者血浆F1+2、D-二聚体浓度显著升高,表明脑梗死患者存在血液高凝状态,为丹参等抗凝治疗提供了理论依据。(2)凝血及纤溶激活分子标志物检测,有助于脑梗死早期诊断,为早期溶栓治疗提供帮助。(3)血浆分子标志物水平对判断梗死范围有一定价值。Abstract: Objective To investigate the changeable law of the levels of plasma Fl + 2 and D-dimer in patients with ischemic cerebral infarction.and the clinical research with radix Salviae miltiorrhizae anticoagulation. Methods In 44 patients with ischemic cerebral infarction, plasma Fl + 2 and D-dimer levels were determined by enzyme-linked immunosorbent assays(ELISA). meanwhile,effect of radix Salviae miltiorrhizae was observed during treatment,6 of which were dynamic determined. Results (l)The levels of Fl + 2 and D-dimer in patients with ischemic cerebal infarction were significantly higher than normal controls(P<0. 01). (2)The levels of plasma F1 + 2 were significantly higher in patients with ischemic cerebral infarction before treatment and dropped to normal after 7 days. The levels of plasma D-dimer in patients with ischemic cerebral infarction elevated before treatment and dropped to normal after 21 days. (3)There is a significant difference between the levels Fl + 2 and D-dimer in patients with multi/big ischemic cerebral infarction and those in patients with lacunar infarction. (4)The abnormal rate of Fl + 2 levels in plasma of patients with ischemic cerebral infarction were in accordance with that of D-dimer (P>0. 05)and both were higher than the rate of CT positive. Conclusion (l)Mean plasma levels of Fl + 2 and D-dimer were significantly higher in patients with ischemic cerebral infarction than that of healthy controls.it suggest a hypercoagulated state in patients with ischemic cerebral infarction and gets theory foundation for radix Salviae miltiorrhizae anticoagulation. (2)determination of markers is helpful for early diagnosis in the ischemic cerebral infarction. It benefit to early phase thrombolysis therapy. (3)The levels of markers can distinguish lacunar cerebral infraction group from multi/big cerebral infarction.
