北京地区精神分裂症患者家属情感表达方式及特征

摘　　要：目的 :了解家属与患者之间的关系 ,探讨北京地区住院精神分裂症患者家属情感表达的种类、对患者的监护水平、对精神疾病认识和受歧视的体验等。方法 :采用包括十个维度的费氏CFI -CV情感表达测评工具 ,对 2 6 5位精神分裂症患者家庭中的 42 7名亲属 ,在患者住院当时进行访谈和录音。访谈后由测评员按标准对各维度进行评分 ,并把被试者划分为高或低情感表达的种类。结果 :42 7名家属中 ,6 1人( 14 3% )对患者有高批评 ,2 6人 ( 6 0 % )表露敌对情感 ,172人 ( 4 0 3% )有情感过分参入 ,总计 2 0 0人( 4 6 8% )有高情感表达。有情感过分参入和高度赞扬的女性明显多于男性。家属对患者在急性期与缓解期治疗的监护方式以被动为主 ,在日常生活和患者对外关系中以主动方式为主。结论 :北京地区住院精神分裂症患者的家属中有高情感表达的比例。Objective: To further understand the relationship between patients and their family members, this study assessed the expressed emotion, care-taking methods, knowledge of mental illness and experience of stigma among family member of hospitalized schizophrenics in Beijing area Method: 427 family members of 265 schizophrenic inpatients were assessed with Philips' 10-subscale Chinese version of the Camberwell Family Interview (CFI-CV) at the time the patient was admitted The audio-taped interview was then independently rated by the examiner and another independent researcher according to standardized criteria All 10 subscales were scored and the respondent was classified as with 'high' or 'low' expressed emotion Results:61 of the 427 family members (14 3%) had high criticism, 26 (6 0%) were hostile and 172 (40 3%) had high emotional over-involvement Overall, 200 respondents (46 8%) were rated as with high expressed emotion Compared with male respondents, female were more likely to have high emotional over-involvement and had more frequent positive remarks During both acute and recovery phases of the illness, family members actively cared for patients' daily needs and actively managed patients' social relationship, but more passive in management of patients' treatment Their knowledge about schizophrenia was minimal They reported that the patient experienced high level of stigma Conclusion:The proportion of family members of schizophrenia with high expressed emotion is similar to their counterparts in the West, but somewhat lower than other reports in China schizophrenia
