摘 要:目的 寻找影响咬合特征的骨骼因素 ,并确定其影响程度 ,为探讨错形成机制以及临床诊断、设计提供理论依据。方法 随机抽取 12 - 15岁间各类错患者共 4 4 0名 ,按牙模型上磨牙关系分为安氏Ⅰ、Ⅱ、Ⅲ类 ,其中安氏Ⅰ、Ⅱ类错者各 16 0名 ,安氏Ⅲ错者 12 0名 ,男女各半。另选择 12 - 15岁正常恒牙者共 6 0例做对照 ,男女各半。在覆、覆盖、磨牙关系与充分反映颅面骨骼特征的 5 6项指标间做多元逐步回归分析。结果 安氏Ⅲ类错型受骨型的影响最大 ,Ⅱ类错覆覆盖受骨型变化的影响最小 ;影响错畸形型的骨骼因素主要包括上下颌矢状关系、垂直骨型、颏部倾斜度。颌凸角与覆盖的变化关系最密切 ,下面高是影响覆的最重要的骨骼因素。结论 骨型对型的影响有限 ,且对安氏各类错的影响程度不同 。Objective This research is to find out the skeletal factors which have influence on the occlusal features and to provide scientific basis for chinical diagnosis, treatment planning and understanding the malocclusion mechanism. Method 440 patients aged from 12-15 years were divided into three groups according to the molar relationship on the plaster model, including 160 children in both class Ⅰ and class Ⅱ groups and 120 children in class Ⅲ group. The control group consisted of 60 normal children in the same age range.Boys and girls are equal in number in all groups. The stepwise multiple regression was performed. Results Skeletal pattern had strongest influence on class Ⅲ occlusal features. The overbite and overjet in class Ⅱ group had the weakest influence on skeletal pattern; The main skeletal factors influencing malocclusion include the saggital and vertical skeletal relationship and the gradient of chin; The NA PoA angel is the most influential factor causing the variation of overjet, while the lower facial height was the most important skeletal factor affecting the variation of overbite. Conclusion Skeletal patten has limited influence on the occlusal pattern, and the extent of it′s influence on the three classes of Angle′s malocclusion is different. Clinically, skeletal pattern and occlusal partern must be considered respectively.
