摘　　要：目的:采用CT与彩色多普勒血流显像(CDFI)联合法对胰腺癌的可切除性进行术前前瞻性评价,以确定其准确性及临床应用价值。材料与方法:1998～2000年经手术及病理证实的43例胰腺癌仅行CT检查40例,CDFI检查36例,其中33例行CT和CDFI联合检查。43例胰腺癌的可切除性评估由两名影像专家进行分析,以手术及病理结果作为标准,评价联合法的诊断价值。结果:43例胰腺癌,经手术探查和病理诊断,14例可根治切除,29例不可根治切除。CT、CDFI及CT与CDFI联合法评估的准确性分别为77.50%,77.78%,87.88%。故在评价胰腺癌可切除性方面联合法的准确性优于单一法。结论:本研究表明,在对胰腺癌可切除性的评估中,CT扫描与CDFI联合法弥补了彼此检查法的不足,能够更为准确、全面地评价胰腺癌的可切除性,具有较高的价值。Objective:To evaluate the use of CT combined with color Doppler flow imaging (CDFI)in preoperative as-sessment of the resectability of pancreatic cancer and to define their accuracy and clinical value.Materials and Methods:43patients with pancreatic cancer confirmed by surgery and histopathology between1998and2000were included.40patients were given CT scans,36patients were given CDFI,in whom33patients took CDFI before CT.The resectability of 43pa-tients with pancreatic cancer was evaluated by two radiologists.CT and CDFI radiographs were correlated with surgical and histopathologic results to assess the clinical value of the combination.Results:Confirmed by surgery and histopathology,14of 43patients had resectable disease.For the other29cases,the lesion was unresectable.Using CT,CDFI and the combination of the two methods,the accuracy were77.50%,77,78%and87.88%,respectively.Therefore,the combination of two modali-ties showed superiority to CT or CDFI in assessing the resectability of pancreatic cancer.Conclusions:In assessing of re-sectability of pancreatic cancer,the combination of CT and CDFI played a complemental role to each other.The combination can further improve accuracy of assessment and have better clinical value.
