不同治疗方案对慢性胃炎幽门螺杆菌根除及治疗效果临床研究

摘　　要：目的:探讨不同治疗方案对慢性胃炎治疗效果和对幽门螺杆菌的根除情况。方法:①符合慢性胃炎诊断标准93例,随机分为治疗组50例,对照组43例;②方案:治疗组(枸橼酸铋钾0.44g/d+克拉霉素0.5g/d+替硝唑1.0g/d)×7d;对照组(枸橼酸铋钾0.44g/d+四环素1.0g+甲硝唑0.8g/d)×7d;②治疗前、后分别钳取胃底、胃体.胃小弯、胃大弯部位组织作Warthin-Starry染色查HP,HE染色判断胃炎程度。观察上腹部疼痛、嗳气、恶心、嘈杂、不适等表现改善。结果:①治疗组治疗前后胃底、胃体、胃小弯、胃大弯Hp根除均有显著意义,均P<0.01,而对照组治疗后对比Hp根除无明显改变。均P>0.05;③治疗组治疗前后胃底、胃体、胃小弯、胃大弯炎症改善比较显著,均P<0.05,与对照组治疗后对比炎症无明显改善,均P>0.05;③临床上腹部疼痛、嗳气、恶心、嘈杂、不适治疗组与对照组总治疗后临床改善无明显差异(X^2=0.1117,P>0.005)。结论:枸橼酸铋钾+克拉霉素+替硝唑方案与世界胃肠病大会推荐方案治疗效果一致,且耐药低、副作用少,可作为一线治疗方案向全国推荐。Objective:To compare the clinical effect between 2 groups with different medical treatment of chronic gastritis and eradication of the Campy lobacter pylori. Methods: ①Ninety three cases with chronic gastritis were enrolled.They were randomly divided into two groups, test group. 50 cases and the ontrol group,43 cases; ② Scheme:Care group given bismuth potassium citrate 0.44g/d+clarithromycin 0.5g/d+tinidazole 1.0g/d × 7d. Control group given bisumth potassium citrate 0.44g/d+tetracycline 1g+metronidazole 0.8g/d × 7d; ③ griped tissue before and after treatment from pars pylorica. corpus ventriculi, curvatura ventriculi minor,curvatura ventriculi major, dyed with Warthin-Starry to find C. pylori and dyed with HE to judge the extent of chronis gastritis. ① Observe the improvement of feeling such as abdominal pain,disgusting and so on. Results; ①The rate of eradication of C pylori before and after treatment in fundus ventriculi corpus ventriculi,curvatura ventriculi minor and curvatura ventriculi major is more significantly in test group. than in control one; ② The rate of improvement of gastritis more before and after treatment is signi!icantly(P<0.05), than the other one (P>0.05); ③ In two groups the improvement of feeling such as abdominal pain,disgusting and so on is unremarkable, (s2=0.1117, P> 0.05). Conclusions:The clinical effect of bismuth potassium citrate + clarithromyein + tinidazole is equal to the prescription which recommended by Warld Gastroenterology Congress, no serious side effects were found, well tolerated, be worth recommending to the doctors all over the country.
