抗HP三联法对患者免疫功能和胃粘膜组织学及胃泌素的影响

摘　　要：目的 探讨抗幽门螺旋杆菌(HP)三联疗法对HP阳性十二指肠溃疡患者外周血T淋巴细胞亚群、胃粘膜病理学及胃泌素含量的影响及其机制。方法 采用碱性磷酸酶-抗碱性磷酸酶复合物(APAAP)方法及放免法对32例HP阳性患者进行外周血T淋巴细胞水平及胃液胃泌素含量检测,胃镜下取活组织常规做尿素酶试验和病理学检查,并与20例HP阴性患者进行对照。结果 治疗后,HP阳性十二指肠溃疡患者外周血CD3、CD4、CD4/CD8比值明显上升,根治HP后胃粘膜中性粒细胞浸润消失,血清胃泌素下降至正常水平。结论 抗HP三联疗法可以改善HP阳性十二指肠溃疡患者胃粘膜的急性炎症反应,并促进其免疫功能的恢复及降低胃泌素的水平。Objective To investigate the influence and mechanism of anti helicobacter pylori ( HP) triple therapy on lymphocyte subpopulation, pathology of gastric mucosa and gastrin concentraion in blood from patients with duodenal ulcer. Methods Alkaline phosphatse-anti alkaline phosphatse complex method and radioimmunoassay were applied to determine T lymphocyte in blood and gastrin concenteration in gastric juices from 32 patients with HP positivity before and after the treatment. Biopsies were taken through gastroscopy and the tissues were stained with urase. The results were compared with those from 20 patients of HP negativity. Results The number of CD3 , CD4 and CD4/CD8 ratio were markedly increased after the treatment. When HP was got cured once and for all, neutrophils could not be found in gastric mocosa; serum gastrin concentration dropped to normal. Conclusions Triple anti-HP therapy could alleviate acute inflammatary reactions in gastric mucosa of DU patients, promoting recovery of immune function and decrease gastrin concentraiton.
