摘　　要：目的评价对于累及Ⅲ区的巩膜穿孔伤进行一期巩膜裂伤缝合联合巩膜扣带的治疗效果。方法回顾性分析2016年1月至2017年8月本院的8例(8眼)巩膜穿孔伤一期巩膜伤口缝合联合巩膜扣带术治疗的临床资料。观察分析术中玻璃体腔填充物,伤后6个月视力、眼压及视网膜情况。结果6例行玻璃体切除术,其中3例填充硅油,术后3个月取出硅油,另3例填充空气。随访期间8例视网膜均在位,视力优于治疗前,差异有统计学意义(t=2.53,P=0.04)。伤后6个月术眼眼压与对侧正常眼眼压差异无统计学意义(t=1.07,P=0.32)。结论一期巩膜扣带术联合巩膜裂伤缝合是治疗累及Ⅲ区的巩膜穿孔伤的有效方法。Objective To evaluate the efficacy of suture of scleral wound combined with primary scleral buckling on scleral perforating injury of Ⅲ region.Methods The data of 8eyes of 8cases with scleral perforating injury of Ⅲ region from Jan.2016 to Aug.2017 were analyzed retrospectively.All patients received primary suture of scleral wound combined with scleral buckling.The followed up time was 6 months after injury.The best corrected visual acuity (BCVA),intraocular pressure and condition of retina 6 months after injury were analyzed retrospectively.Results Six cases underwent vitrectomy,among them 3 cases received silicone oil tamponade and 3 cases with air tamponade.The silicone oil was removed 3 months later.The retina at 6 months after injury was flat in 8 cases.The visual acuity after surgery was better than that before treatment (t =2.53,P =0.04).The difference of intraocular pressure was not statistically significant between operated eyes and contralateral eyes at 6 months after injury (t =1.07,P =0.32). Conclusion Suture of scleral wound combined with primary scleral buckling is effective for scleral perforating injury of Ⅲ region.
