摘  要:目的探讨超声弹性成像及微血管成像在乳腺癌良恶性肿块中的诊断价值。方法应用常规超声结合弹性成像及微血管成像对86例乳腺肿块患者87个肿块进行检查,以病理结果作为对照并分析。结果恶性肿块以弹性评分3~4分为主;弹性应变率比值阈值为2. 80,微血管成像中血流分级以3级为主。弹性评分诊断乳腺肿块良恶性的敏感性、特异性、准确性三者分别为71. 4%、92. 1%、80. 5%,而弹性应变率比值诊断分别为89. 8%、90. 8%、92. 1%,微血管成像诊断分别为67. 3%、50%、59. 8%,常规彩色多普勒技术诊断分别为65. 3%、41. 6%、49. 4%,检查技术准确性比较,差异有统计学意义。结论弹性成像联合微血管成像可作为评估乳腺肿块良恶性的有效方法,在对乳腺肿块良恶性诊断中有良好的临床应用价值。Objective To investigate the diagnostic value of ultrasound elastography and microvascular imaging in benign and malignant breast cancer.Methods Elastography and microvaseular imaging were used to detect 49malignant and 38benign lesions in 87lumps of 86breast cancer patients.Results The malignant masses were mainly elastic score 3-4,the elastic strain rate ratio threshold was 2.80,and the blood flow grade in the mieroangiography was 3.The sensitivity,specificity and accuracy of elastic scores in the diagnosis of benign and malignant breast were 71.4%,92.1% and 80.5%respectively.The sensitivity,specificity and accuracy of elastic strain ratio ratio were 89.8%,90.8%and 92.1%respectively.The sensitivity,specificity and aceuracy of microvaseular imaging in the diagnosis of breast benign and malignant lesions were 67.3%,50%and 59.8%respectively.The difference in the accuracy of the three results was statistically significant.Conclusion Elastography ean be used as an effective method for evaluating benign and malignant breast lesions,and is of good elinical value in the diagnosis of benign and malignant breast masses.
