摘  要:目的探讨非药物方法联合治疗儿童注意缺陷多动障碍(attention deficit hyperactivity disorder, ADHD)的临床疗效。方法选取2017年5月至2018年6月于湖北医药学院附属襄阳市第一人民医院儿童保健科诊治的76例ADHD患儿,采用脑电生物反馈仪、平衡训练仪联合治疗,治疗前后采用脑电生物反馈仪脑电波指标、平衡训练仪参数设定值和视听整合测试分析评估不同表型ADHD患儿的治疗效果和不同疗程的治疗效果。结果 76例患儿治疗20次时,生物反馈仪脑电波指标与治疗前比较,差异有统计学意义(P <0.05);治疗40次后,生物反馈仪脑电波指标与治疗20次时比较,差异有统计学意义(P <0.05)。3种表型ADHD患儿进行平衡训练仪治疗20次时,多动型训练成绩分值逐渐升高,与治疗前比较,差异有统计学意义(P <0.05);注意缺陷型和混合型训练成绩分值无明显变化,与治疗前比较,差异无统计学意义(P> 0.05);3种表型ADHD患儿进行平衡训练仪治疗40次时,多动型、注意缺陷型和混合型训练成绩分值均升高,与治疗前和治疗20次时比较,差异均有统计学的意义(P <0.05)。视听整合连续执行测试(integrated visual and auditory continuous performance test, IVA-CPT)评分,治疗20次时综合尺度反应控制商数评分显著升高,与治疗前比较,差异有统计学意义(P <0.05),综合尺度注意商数与治疗前比较,差异无统计学意义(P> 0.05);治疗40次时综合尺度反应控制商数和综合尺度注意商数评分均显著升高,与治疗20次时比较,差异有统计学意义(P <0.05)。结论对ADHD患儿采用2种非药物方法联合治疗,能有效唤醒其注意力和控制力,改善感觉统合失调,依从性好,无药物不良反应,值得临床推广应用。Objective To explore the clinical effect of combination of non-drug therapy on attention deficit hyperactivity disorder(ADHD) in children. Methods Seventy-six children with ADHD were treated with biofeedback and balance training. The parameters of electroencephalogram(EEG) biofeedback, balance training and integrated visual and auditory test were collected before and after treatment to evaluate the effect on different kinds of ADHD. Results When 76 children with ADHD were treated for 20 times, there was statistically significant difference in brain wave parameters between pre-and post-treatment with biofeedback instrument(P<0.05). After 40 times of treatment, there was statistically significant difference in brain wave parameters between treatment with biofeedback instrument for 20 times and that for40 times(P<0.05). After treatment of balance training instrument for 20 times, the scores in hyperactive type of ADHD children gradually increased significantly compared with pre-treatment(P<0.05). There was no significant change in the score of attention deficit type and mixed type of ADHD children(P>0.05). After treated with the balance training instrument for 40 times, the scores in all hyperactive type of ADHD children, attention deficit type and mixed type were significantly increased than those before treatment and after treated for 20 times(P<0.05). In audio-visual integration continuous execution test(IVA test), the comprehensive controlling ability scores significantly increased after 20 times(P<0.05),while the comprehensive attention scores had no statistically significant between pre-treatment and treated for 20 times(P>0.05). But both comprehensive controlling ability and comprehensive attention scores were significantly increased after treated for 40 times when compared with pre-treatment and treated for 20 times(all P<0.05).
