伽玛刀治疗原发性三叉神经痛焦点剂量率的临床研究

摘　　要：目的研究焦点剂量率对伽玛刀治疗原发性三叉神经痛(TN)的影响情况。方法回顾性分析112例经伽玛刀治疗并获得有效随访的原发性三叉神经痛病例资料。以中位焦点剂量率(2.38 Gy/min)为限,将病例分为高焦点剂量率组和低焦点剂量率组,比较两组伽玛刀治疗后有效率、复发率、起效时间及并发症发生率等。结果在伽玛刀治疗后,治疗有效率、复发率与并发症发生率两组间差异均无统计学意义(P> 0.05),而高焦点剂量率组中位起效时间早于低焦点剂量率组,差异具有统计学意义(P <0.05)。结论焦点剂量率在1.50～3.34 Gy/min范围内,其改变不会对伽玛刀治疗原发性三叉神经痛的有效率、复发率及并发症发生率产生影响,但起效时间会随焦点剂量降低而延长。Objective To study focus dose rate of gamma knife for primary trigeminal neuralgia.Methods The clinical data of 112 patients with primary trigeminal neuralgia were analyzed retrospectively.All the patients were treated with gamma knife and obtained effective follow-up.According to the median focus dose rate (2.38Gy/min),the patients were divided into high focus dose rate group and low focus dose rate group.The response rate,recurrence rate,onset time and complication rate were compared between the two groups.Results After gamma knife radiosurgery,the response rate,recurrence rate and complication rate showed no statistical differences between the two groups (P>0.05).The median onset time in high focus dose rate group was earlier than that in low focus dose rate group,and the difference was statistically significant (P <0.05).Conclusions In the range of 1.50-3.34Gy/min,the change of focus dose rate could not affect the response rate,recurrence rate and complication rate of gamma knife for primary trigeminal neuralgia.But the onset time will be prolonged with the decrease of focus dose rate.
