益气和中方加减治疗慢性萎缩性胃炎(脾胃虚寒证)临床疗效研究及作用机制分析

摘　　要：目的:观察益气和中方治疗脾胃虚寒型慢性萎缩性胃炎的临床疗效及对其作用机制进行初步探讨。方法:将来本院收治的92例慢性萎缩性胃炎患者,随机分为2组,对照组患者口服雷贝拉唑肠溶胶囊和阿莫西林胶囊,1个月为1疗程,连续治疗3个疗程。治疗组采用益气和中方治疗,每日1剂,分早晚2次温服,1个月为1疗程,连续治疗3疗程。检测比较治疗前后血清中胃蛋白酶原Ⅰ(PGⅠ),血清胃泌素-17 (G-17)及血浆胃动素(MOT)的水平,比较2组治疗后中医症候及体征积分、胃黏膜组织病理评分及临床疗效。结果:2组患者治疗后PGⅠ、G-17及MOT水平均显著升高(P<0. 05),并且治疗组在治疗后的PGⅠ、G-17及MOT水平显著高于较对照组(P<0. 05);治疗后治疗组的中医症候及体征积分及胃黏膜组织病理评分均显著高于对照组(P<0. 05); 2组临床疗效比较,对照组的总有效率为82. 2%,治疗组的治疗总有效率为93. 6%,差异显著(P<0. 05)。结论:益气和中方治疗慢性萎缩性胃炎可以显著提高患者PGⅠ、G-17及MOT水平,临床症状改善明显,疗效显著。Objective: To study the clinical efficacy and mechanism of using Modified Yiqi Zhonghe Recipe in the treatment of chronic atrophic gastritis with deficiency of spleen and stomach. Methods: 92 patients with chronic atrophic gastritis in our hospital were randomly divided into 2 groups. The controlled group was treated with Bella Enteric Coated Capsules and Amoxicillin Capsules,1 months for 1 courses,and 3 courses of continuous treatment. The treatment group treated by Modified Yiqi Zhonghe Recipe,1 day and the 2 day,1 month to 1 course of treatment,continuous treatment of 3 courses. The levels of pepsinogen I( PG I),serum gastrin-17( G-17) and plasma motilin( MOT) before and after treatment were detected and compared. The scores of TCM symptoms and signs,the pathological score of gastric mucosa and the clinical efficacy were compared between the 2 groups after treatment. Results: 2 groups of patients after treatment of PG I,G-17 and MOT levels were significantly increased( P < 0.05),and in the treatment group after treatment of PG I and G-17 and MOT levels were significantly higher than the controlled group( P < 0. 05); After the treatment of TCM symptoms and signs score and gastric mucosa pathological assessment of the treatment group there were significantly higher than those in the controlled group( P < 0. 05); To compare the clinical efficacy of the2 groups,the total effective rate of the controlled group was 82. 2%,the total effective rate of the treatment group was 93. 6%,had significant difference( P < 0. 05). Conclusion: Using Modified Yiqi Zhonghe Recipe in the treatment of chronic atrophic gastritis can significantly improve the level of PG I,G-17 and MOT in patients with clinical symptoms improved significantly,curative effect is remarkable.
