摘  要:目的了解学龄前儿童碘营养与体重指数状况,为进一步做好学龄前儿童肥胖防治工作提供科学依据。方法采用整群抽样,对湖北省郧西县县城内5所幼儿园的3~6岁学龄前儿童进行晨尿碘和血促甲状腺激素(TSH)测定,测量身高和体重。结果收集儿童晨尿和血清标本各944份,尿碘中位数为191.3μg/L,男生、女生尿碘中位数分别是201.5μg/L和188.0μg/L。3~、4~、5~、6岁~各年龄组尿碘中位数分别是185.3、190.4、192.0和210.5μg/L,不同性别间尿碘水平差异有统计学意义(P<0.05)、不同年龄组间尿碘水平构成差异无统计学意义(P>0.05)。TSH中位数为2.67μIU/ml,升高率为14.6%。3岁以上年龄组TSH水平和升高率与3岁组相比差异有统计学意义(P<0.05)。儿童超重率为10.1%,肥胖率为5.1%。Logistic回归分析发现学龄前儿童超足量碘和过量碘是超重肥胖的危险因素(P<0.05),OR值(95%CI)分别为1.80(1.089,2.975)和2.02(1.173,3.470)。结论有较大比例的学龄前儿童的尿碘处于超足量和过量,而超足量以上碘营养水平可增大儿童超重肥胖的危险性。Objective To explore the preschool children’s status of iodine nutrition and body mass index, and to provide scientific basis for further prevention and control of obesity in preschool children. Methods Cluster sampling was used to select randomly 5 nursery schools of Yunxi County, Hubei Province. Morning urine iodine and blood samples were taken in the selected 3-to-6 year-old children. Thyroid stimulating hormone(TSH), height and weight were measured. Results A total of 944 children’s morning urine and serum samples were collected. The median urinary iodine was 191.3 μg/L in the children and the median urinary iodine was 201.5 μg/L and 188.0 μg/L for the boys and the girls, respectively. The median urinary iodine levels in the children aged 3, 4, 5, and 6 years-old were 185.3 μg/L, 190.4 μg/L, 192.0 μg/L, and 210.5 μg/L, respectively. There was a significant difference in urinary iodine levels between children of different genders( P<0.05), and there was no significant difference in urinary iodine levels among different age groups(P>0.05). The median TSH was 2.67μIU/ml, and the rate of abnormal higher level of TSH was 14.6%. The level of TSH and the rate of increase in the age group over 3 year-old were significantly different from those in the 3 year-old group(P<0.05). The rate of overweight children was10.1%, and the obesity rate was 5.1%. Logistic regression analysis revealed that more than adequate and excess iodine in preschool children were risk factors for overweight and obesity(P<0.05), and the OR(95% CI) were 1.80(1.089,2.975) and2.02(1.173,3.470). Conclusion A large proportion of preschool children’s urine iodine level is more than adequate and in excess, and this may be the risk of overweight and obesity in children.
